A lot of ways to look into phenotype in ARDS. One of the ways is to check the inflammation status.
Hypo versus hyper inflammatory subphenotype.

But why?

Hyperinflammatory subphenotype is associated with
⬆️ mortality
⬆️PEEP requirement
⬆️ chances of RV dysfunction
⬇️ ventilator free days
Also more chances of association with extrapulmonary sepsis and (may be) simvastatin might work.

The adverse events associated with physical restraint use:
- Skin injuries
- Subsequent delirium
- Neuro functional decline
- Post traumatic stress disorder.
Also there is not much evidence that it prevents unplanned extubations and device removal.
Although high quality evidence is lacking, would you like to use physical restraints in ICU?

Augmented Renal Clearance (ARC)

Definition: GFR more than 130ml/min/1.73 m2
Pathophysiology: combination of systemic inflammation and activation of renal reserve
Labs: 24 hour urinary CrCl is gold standard
Risk factors: Young age, Trauma, Head injury, Burns, Sepsis
Impact: Underdosing of commonly used medications like beta lactams, vancomycin, levetiracetam etc
Onset: 50% within first three days and 50% within first week.
Duration: highly variable

Looks simple. But complicated. You need to consider a lot of factors in the interpretation. More importantly, it can be normal or high in sepsis.

Tip: Make sure you study the CO2 dissociation curve before attempting to understand this concept.
