How do they work?
How much do we understand Mckenzie approach?
We know that the well-known Mckenzie Lx extension exercise for posterior disc derangement.
For example, once there is a tear in the annular fibrosus, the material from the nucleus pulposus shifts posteriorly through the tear, especially more during Lx flexion such as bending forward and sitting. This is called "discogenic pain".
Therefore, how we understand about Mckenzie approach is that Lx extension can shift the material from the nucleus pulposus anteriorly. some patients with discogenic pain may respond to this well.
Mckenzie recommends extension exercises in the treatment of posterior disc derangement because of stress reduction on the pain-sensitive posterior annulus due to anterior displacement of the nucleus.
Yes. This is his theory. However, does this really occur to everyone with the posterior disc derangement?
What if the extension exercises worsen the discogenic pain?
This worse pain is commonly thought due to degenerative spinal joints like spinal stenosis.
HOWEVER,
Research!!
Greater disc bulging under postural loading occurs with advancing degenerative disc disease. Increasing bulging was noted at lower spinal levels (L4–L5 and L5–S1)
Grade 1 discs displayed the expected response to dynamic positions. However, more degenerative discs behave less predictably, and EXTENSION may result in significant POSTERIOR disc bulging.
These results question the popular therapeutic techniques.
I am not against Mckenzie exercises. Some patients respond to them well.
This research clearly proves that patients should be individually assessed and treated by pathomechanism rather than focusing on pathology.
If pathology (disc herniation) is focused, then we may prescribe the Mckenzie extension exercises to all patients with disc herniation. Therefore, focusing on pathomechanism may be more appropriate.
