EPILEPSY SURGERY: LEFT FUNCTIONAL HEMISPHEROTOMY FOR RASMUSSENS ENCEPHALITIS

12 yearboy with right spastic hemiplegia and epilepsy since last 9 years. Speech comprehension preserved and expression partial.

EEG showed left cerebral diffuse epileptic discharges
MRI showed left cerebral damage and gliosis.
Functional MRI showed speech signals on the right frontal lobe and bilateral temporal lobes.

He underwent DISCONNECTION TYPE OF EPILEPSY SURGERY. Functional hemispherotomy was performed disconnecting the dysfunctional epileptogenic cerebral hemisphere from the rest of the brain.

Commissural and Projection fibres were cut.
Hippocampal disconnection was done by cutting the tail near the trigone.

Postoperatively he had NO seizures and NO new neurological deficits.
