A,71 years old lady, is a follow up case of Malignant phylloides tumor right breast post surgery on 22.03.2019, post adjuvent radiotherapy was present in OPD with complaints of ride side chest pain, low grade fever, loss of appetite with breathing difficulty since 1 month. Routine investigations were normal. Her chest x-ray reveled Right upper zone consolidation. For further evaluation ct chest then after PET CT was done which revealed metabolic active soft tissue mass lesion noted in right lung upper lobe infiltrating mediastinum abutting trachea (5.5 x 6.1 x 6.2 cm) associated with metabolically active soft tissue mass noted in right breast( 1.1 x1.5 cm) ? Mitotic. Lumpectomy was done which reveled old fibrotic tissue and bronchoscopy guided was done which reveled INVASIVE ASPERGILLOSIS.
