Case Summary:
A middle-aged woman presented with intermittent severe headaches. Imaging revealed a large left supraclinoid ICA aneurysm and a small clinoidal aneurysm—both incidentally detected. She was otherwise asymptomatic, raising questions about the aneurysm’s link to her symptoms.

Key Clinical Decisions:
✅ Need for treatment of the large supraclinoid aneurysm
✅ Management of the small clinoidal aneurysm (treat or observe)
✅ Balancing intervention risks versus rupture or growth

Management & Outcome:
Cerebral DSA confirmed aneurysm anatomy and guided treatment planning. A reconstructive endovascular approach was chosen — flow diverter placement covering both aneurysms, supplemented with coiling of the large aneurysm.
The patient tolerated the procedure well, with no postoperative neurological deficits.

Rationale:
Flow diversion facilitates vessel reconstruction and aneurysm occlusion, while adjunctive coiling enhances immediate closure and reduces thrombosis risk.
This technique has transformed the management of large or complex intracranial aneurysms, offering a safer alternative to surgical clipping or bypass.

Current Indications for Flow Diverters:
➤ Large or Giant Aneurysms
➤ Wide-Neck Bifurcation Aneurysms
➤ Fusiform or Blister Aneurysms

Recent Advancements:
🔹 Surface Modification: Newer devices with phosphorylcholine (PC) coating reduce thrombogenicity and improve biocompatibility.
🔹 Low-Profile Designs: Models like FRED Jr enable treatment of smaller or distal aneurysms.
🔹 Bioresorbable Flow Diverters: Being developed to minimize long-term complications.

Outcomes:
Flow diverters achieve 70–90% aneurysm occlusion rates with low complication risks such as thromboembolic events or hemorrhage.
Close imaging follow-up is essential to assess occlusion and vessel patency.

Conclusion:
This successful case highlights the safety and efficacy of flow diversion in managing complex intracranial aneurysms, providing durable results with minimal morbidity.
