In cases of non-tuberculous mycobacteria (NTM) infection, the acid-fast bacilli (AFB) or Ziehl–Neelsen (ZN) stain of a sputum sample may yield a positive result because NTM organisms also retain the stain due to their mycolic acid-rich cell walls. However, the interferon-gamma release assays (IGRAs), such as the QuantiFERON-TB test, which are designed to detect an immune response specific to Mycobacterium tuberculosis complex antigens, are typically negative in NTM infections, as these antigens are absent in most NTM species.
