An interesting case...
A **na in a 67 year old lady .... a Right Inguinal Lymph Node was FNA positive and mild FDG uptake in Bilateral Pelvic Lymph Nodes.
All walls of the Va**na were involved right upto the Cervix .
I did a Radical Hysterectomy with a Radical VulvoVaginectomy IN CONTINUITY .... along with a Bilateral InguinoPelvic Lymphadenectomy .
I've done many Anterior , Posterior and Complete Pelvic Exenterations and of course, hundreds of Radical Hysterectomies .... but somehow hadn't done this type of procedure previously. ( Which is actually quite odd since I've operated almost 12,000 Oncosurgery cases till now ) ....
The Challenge was ensuring no injuries to the Re**um or the Bladder .... while removing all the organs in continuity and with safe margins.
Total Operative Time : 3 hours .
Blood Loss : 200ml .
R-0 Resection achieved.
