यह 56 वर्षीय सज्जन चलने और बोलने में लगातार कठिनाई, चेहरे के बाईं ओर कमज़ोरी, बाईं आँख बंद न कर पाना, मुँह का दाईं ओर मुड़ जाना, खाने और निगलने में कठिनाई और खाते-पीते समय खांसी की शिकायत लेकर आए थे।
कंट्रास्ट के साथ मस्तिष्क की लगातार एमआरआई रिपोर्ट में एक बड़ा ब्रेन ट्यूमर दिखाई दे रहा था जो सेरेबेलो पोंटीन कोण नामक पश्च फोसा के बाईं ओर से फैल रहा था और ट्यूमर पूरी तरह से ट्यूमर से घिरा हुआ था। ट्यूमर जुगुलर फोरामेन के माध्यम से गर्दन तक भी फैल रहा था। सेरेबेलो पोंटीन कोण मस्तिष्क के उस क्षेत्र के करीब होता है जो चेतना, श्वास, हृदय, निगलने, खाने और चेहरे की गतिविधियों जैसे महत्वपूर्ण कार्यों को नियंत्रित करता है - अगर मानव शरीर में आत्मा नाम की कोई चीज़ है, तो यह क्षेत्र इतना महत्वपूर्ण है कि माना जाता है कि आत्मा इसी क्षेत्र में निवास करती है।
ऐसे क्षेत्र में सर्जरी बेहद जोखिम भरी होती है, लेकिन ऐसे ट्यूमर से जान बचाने का यही एकमात्र तरीका है।
ट्यूमर के स्थान और विस्तार को देखते हुए, ऐसा प्रतीत होता है कि यह नौवीं, दसवीं या ग्यारहवीं तंत्रिका से उत्पन्न हुआ है। ये तंत्रिकाएँ निगलने, खाने, हृदय गति और गर्दन व सिर की गति को नियंत्रित करती हैं।
चूँकि इस ट्यूमर का पहले ही विकिरण उपचार हो चुका था, इसलिए सर्जरी ज़्यादा जोखिम भरी मानी जा रही थी।
सर्जरी की ज़रूरत, इसकी उपयोगिता, लाभ, सीमाएँ और संभावित जटिलताएँ, जिनमें लंबे समय तक बेहोशी, साँस लेने में तकलीफ़ और खांसी में संभावित वृद्धि, वेंटिलेशन और आईसीयू में रहने की ज़रूरत शामिल है, मरीज़ और उसके रिश्तेदारों को ऑपरेशन से पहले की चर्चा और तैयारी के तौर पर ट्रेकियोस्टोमी के बारे में समझाया गया, जिसमें सभी ज़रूरी जाँचें भी शामिल थीं।
उनकी सहमति के बाद, बाईं ओर के मस्तिष्क और रीढ़ की सर्जरी की गई।
सर्जरी के तुरंत बाद मरीज़ होश में आ गया। उसकी हालत में सुधार होने लगा।
ऑपरेशन के दूसरे दिन उसे बिस्तर से उठा लिया गया और तीसरे दिन घर भेज दिया गया।
अब तक के इतिहास में दुनिया भर से केवल 44 ऐसे ट्यूमर के मामले सामने आए हैं

Large Brain Tumor at important location called causing Walking, breathing and eating difficulty - was treated earlier with but had continued to increase in size and progressively cause difficulty in daily living - removed totally giving a new lease of life to the 56 year man.
This 56 year gentleman came with progressive difficulty in walking, speaking, weakness on the left side of face, inability to close the left eye, deviation of mouth towards right, difficulty in eating and swallowing and coughing on eating and drinking
Serial MRI Brain with contrast was showing an increasing size Large Brain Tumor extending from left side of the posterior fossa called cerebello pontine angle which was fully occupied by the tumour and the tumour was also extending to the neck through the jugular foramen. Cerebello pontine angle is close to the area of Brain which controls the important functions like consciousness, breathing, heart, swallowing, eating, face movements - if there is something called soul in human body, this area is so important that the soul is supposed to be living residing in the region
Surgery in such area is highly risky but that's the only way to save life from such tumours
Looking at the location and extent of the tumour, it appeared to be originating from the ninth, tenth or 11th nerve. These nerves control swallowing, eating, heart rate and movement of neck and head.
As this tumor had received radiation therapy earlier, surgery was supposed to be more risky
Need for surgery, it's usefulness, benefits, limitations and possible complications including prolonged unconsciousness, possible increase in difficulty in breathing and cough and need for ventilation and ICU stay, Tracheostomy was explained to the patient and relatives as part of pre operative discussion and preparation including all necessary investigations
After their consent, left side brain and Spine surgery was done
Patient became conscious immediately after surgery
He started improving
He was mobilised out of bed on second post operative day and sent home on third day
This type of surgery is a team work with other departments and Neurosurgery and needs good technical and ICU support in addition to good surgical skills
