Most of the chronic head & neck pains, largely treated as Migraine are basically a very common postural pain commonly known as "Myofascial Pain".

Most of the headaches can be cured, if attended on time and properly. Mostly any pain localized to temporo-occipital region is being treated as migraine headache, whereas most of these cases are myofascial in origin. Myofascial pain responds beautifully if treated properly.
So, it is very important to identify the problem properly and on time.

Cervical Myofascial Pain originates from the vertebral spine in the neck correlating to muscle and its surrounding fascia (sheath of connective tissue supporting or binding together internal organs or parts of the body.). The diagnosis of this syndrome in clinical, with no confirmatory laboratory tests available. Thus, myofascial pain in any location is characterized on examination by the presence of trigger points located in skeletal muscle. In the cervical spine, the muscles most often implicated in myofascial pain are the trapezius, levator scapulae, rhomboids, supraspinatus, and infraspinatus. A trigger point is defined as a hyperirritable area located in a palpable taut band of muscle fibers.

Trigger points are known to elicit local pain and/or referred pain in a specific recognizable distribution.
Palpation in a rapid fashion (ie, snapping palpation) may elicit a local twitch response (LTR), a brisk contraction of the muscle fibers in or around the taut band. The LTR also can be elicited by rapid insertion of a needle into the trigger point.
Restricted range of motion (ROM) and increased sensitivity to stretch of muscle fibers in a taut band are noted frequently.
The muscle with a trigger point may be weak because of pain. Usually, no atrophic change is observed.
Patients with trigger points may have associated localized autonomic phenomena (eg, vasoconstriction, pilomotor response, ptosis, hypersecretion).
An active myofascial trigger point is a site marked by generation of spontaneous pain or pain in response to movement. This phenomenon is in contrast to the case of latent trigger points, which may not produce pain until they are compressed.

In normal circumstances, body tries itself to contain any painful/stressful lesion by endogenous secretion of Glucocorticoides/Cortisol and that generally happens at subconscious level, which we are unable to identify. As the intensity of pain/stress increases we started perceiving the pain/stress. Continuous pain/stress and subsequent hyper-responsiveness of Sympathetic nervous system and endogenous derangement's in our body are responsible for most of the life-style changes in our body.
So, it is always better to identify these stressful events and get rid of the disease process, instead of controlling the symptoms.

Most of the cases with cervical myelgia with symptomatic appearance as Migraine headache should be thoroughly evaluated for myofascial triggers in the neck. Presentation of myofascial pain is almost as same as migraine headache but in most of the cases patient's are forced to take treatment for life-long.

Myofascial pain can be managed with very less dependency on medications, whereas because of lack of proper approach most of these cases are forced to take treatment for life-long.

Treatment gives cure when approach is good.

Most of the headaches like cervical and lumbar spondylosis (for every neck and back pain) are erroneously over-diagnosed as migraine headache. In maximum number of cases these headaches are because of some latent myofascial triggers in the para-spinal muscles.
Management is quite simple and if managed systematically and efficiently, a permanent cure is very easy to achieve.
Trigger point release followed by stretching exercises is the ultimate cure for this over diagnosed issue. Above all, regular medication is also not required.
It's always good to focus on permanent cure instead of looking for symptomatic relief only.

Repetitive Trans-cranial Magnetic Stimulation for Migraine Headache

TMS stands for transcranial magnetic stimulation. The treatment involves the person holding a device on their scalp that then delivers a magnetic pulse through the skin. The device is portable and can be used at home or wherever is convenient.

The number of pulses can be changed – from a single pulse (sTMS) to repeated pulses (rTMS). The strength, frequency and length of time it is given can also be varied for each individual. The device records the treatments, which can be useful as part of a headache diary.

Migraines are severe headaches that are usually accompanied by additional symptoms including nausea and vomiting, and aversion to light and sound. A migraine may or may not be preceded by a warning “aura” where the person may experience visual changes such as seeing flashing lights or zigzag lines or other symptoms.

The cause of migraines is not known, but involves sudden contraction and then dilation of blood vessels in the brain. Numerous triggers have been identified including changes in hormone levels, environment (such as bright lights), diet and emotions.
