Immediate Implants-

Putting an implant immediately after extraction is another ball game.

Firstly, case assessment amd selection is very important.

Periapical pathologies, bone health, gingival health are important factors.

Secondly, curetting the whole socket throughly to not leave any granulation is top priority.

Thirdly, getting primary stability by going beyond the apex is very important as the socket might be larger than implant, and only primary stability you get is from apical area.

And to my last point, important to always have bone graft and membrane ready as there could be ( and mostly is) jumping distance between implant and native bone.

Good closure and we are good to go!

Medical science isn’t always about success. You win most, but you lose some.

Here is a case of failed implant.
Picture 1 - Post surgery IOPA. Case got finished within an hour without any hassle. Patient was fine. Implants looked awesome. Three days later he started complaining of pain. No associated Parasthesia or numbness. IOPA didn’t reveal anything. Gave antibiotics, analgesics, anti inflammatory meds, still l the patient was uncomfortable. Took guidance of other learned faculty as well, no one could pinpoint at what’s wrong.

Picture 2 - After about 20 days. Finally IOPA revealed peri apically infected implant.
Removed it immediately, curettage done, patient got relief.
Learnings from one failed case is equals to 100 successful cases.

Immediate extraction and Implants.
In case of molars, jumping distance(distance between implant and bone) is usually high due to wide socket, so it’s important to go beyond the socket and find the primary stability from there as noticeable in this radiograph.

I believe Good armamentarium is key to success rate of surgeries.

And Osstem kit is one such friend. Sharp length specific drills and a ratchet with clearly assessable torque values makes it a reliable partner.

Well placed implants though looks beautiful ( to me at-least 😛) but also very important for function as well. Taking care of Bucco- lingual and mesio- distal direction while placing them helps dissipate occlusal forces better and doesn’t stress out the Bone Implant Junction, leading to implant longevity.

IMPLANT MAINTAINENCE-

Implant longevity for patients is not just well placed implant and delivering a good prosthesis. The secret sauce is awareness and recall visits. Hence following Merin’s guidance for SPT, I ask my patients to be in touch and take an IOPA every 6 months in first year, and then every year.

This patient came for her recall visit after an year. Implant was done 2 years back. Looks all nice and tucked in! 😀
Next year reminder already set in her phone by the patient. 😌

DENTAL IMPLANTS IS NOT JUST PLACING IMPLANTS!
A good prosthesis is equally, or in fact more important than the surgical part.
Patient got implants done from somewhere else, came to the clinic from a referral and complained of frequent loosening of prosthesis (3 times in 6 months). Did a diagnostic IOPA, and to my horror, the screw by barely attached to the implant and the angulation was so awful that it wasn’t even going further inside.

Started with the prosthetic part from scratch
Curettage followed by healing screws ➡️ Angulated abutments➡️ New prosthesis
It’s been more than 6 months and the patient is biting on apples and pears!
PS - In my over enthusiasm ( or laziness) , forgot to take pre- procedure clinical pictures 😬
