An old age patient was having sinusitis......
Was receiving good treatment for sinusitis, however headache continued to worsen. Detailed history and clinical examination revealed deep seated sinusitic pattern with dural involvement. MRI brain clearly showed trailing sinusitis from posterior ethmoids into both cavernous sinuses.
Immediate antifungal treatment had dramatic improvement in headache and diplopia.
*Message: some variants of sinusitis can be fatal if diagnosed late
