Severe Oligohydramnios: Emergency Delivery कब जरूरी होती है? | High-Risk Pregnancy | Dr Mannan Gupta, Delhi

इस case में baby के आसपास amniotic fluid (पानी) बहुत कम था, जिसे severe oligohydramnios कहा जाता है। ऐसी condition में baby की safety के लिए timely delivery और close monitoring बेहद जरूरी हो जाती है।

हालांकि ultrasound में किसी major congenital defect के signs नहीं दिखे, लेकिन बहुत कम fluid होने पर baby में breathing issues या lung development problems का risk बढ़ सकता है। इसलिए pediatric team को पहले से alert किया गया और जरूरत पड़ने पर NICU support, oxygen, tube feeding या lung maturity treatment की तैयारी रखी गई।

Baby की heartbeat फिलहाल stable थी, इसलिए mother को IV fluids और lung maturity injections दिए गए और planned emergency C-section की तैयारी की गई। ऐसे cases में सही समय पर decision लेना mother और baby दोनों की safety के लिए बहुत important होता है। Severe oligohydramnios में emergency delivery की जरूरत पड़ सकती है

Natural Labor at 39 Weeks: Normal Delivery के Chances कैसे बढ़ते हैं? | Dr Mannan Gupta, Delhi

कुछ दिन पहले यह patient 38 weeks पर anxiety की वजह से induction of labor करवाना चाहती थीं। लेकिन mother और baby दोनों stable थे, इसलिए unnecessary induction avoid किया गया और patient को wait करने की सलाह दी गई।

आज सुबह patient को naturally leaking शुरू हुई और labor pains अपने आप आने लगे। Examination में baby का head नीचे आ चुका था, NST बिल्कुल normal था और labor धीरे-धीरे अच्छी तरह progress कर रहा था। Cervical gel support के बाद patient active labor में चली गई और normal delivery के अच्छे chances बन गए।

यह case दिखाता है कि अगर mother और baby stable हों, तो कई बार 39–40 weeks तक patience रखने से body naturally labor start कर देती है और normal delivery की संभावना बढ़ जाती है। हर pregnancy अलग होती है, इसलिए timely monitoring और doctor guidance बहुत जरूरी होती है।

Meconium in Pregnancy: Emergency में भी Normal Delivery कैसे Possible हुई? | Dr Mannan Gupta, Delhi

इस case में patient दूसरी बार pregnant थीं और पूरी pregnancy के दौरान regular follow-up में थीं। 37 weeks तक हमने “no unnecessary intervention” policy follow की और patient को सिर्फ warning signs—pain, bleeding, water leakage या baby movements कम होने—पर तुरंत hospital आने की सलाह दी।

जब patient labor pain के साथ hospital पहुंचीं, तो cervix अच्छी तरह dilated थी और baby normal delivery के लिए ready था। लेकिन examination में पता चला कि baby ने meconium pass किया हुआ है और fetal heart rate बार-बार drop हो रही थी, जो emergency sign हो सकता है।

Timely monitoring, oxygen support, IV fluids, augmentation of labor और proper guidance के साथ सिर्फ आधे घंटे में safe normal vaginal delivery कराई गई। Mother और baby दोनों healthy हैं और C-section की जरूरत नहीं पड़ी।

Premature Delivery: CPAP Support क्यों देना पड़ता है? | NICU Care Tips | Dr Mannan Gupta, Delhi

इस case में 33 weeks का premature baby safely deliver हुआ, जिसका weight लगभग 1.8 kg था। Delivery के बाद baby को mild respiratory distress और oxygen saturation fall की problem हुई, इसलिए तुरंत CPAP (Continuous Positive Airway Pressure) support शुरू किया गया।

CPAP therapy premature babies में breathing support देने के लिए इस्तेमाल की जाती है, ताकि lungs properly expand हो सकें और oxygen levels stable रहें। Doctors ने baby को NICU observation में रखा और अगले 24 hours तक closely monitor करने का plan बनाया। अगर जरूरत पड़ती, तो surfactant therapy या ventilator support भी दिया जा सकता था।

इस consultation में pregnancy के दौरान होने वाली amniotic fluid leakage (high leak) के बारे में समझाया गया। कई बार amniotic membrane में छोटा hole हो जाता है, जिससे धीरे-धीरे fluid leak होने लगता है। कुछ cases में यह leak अपने आप seal हो सकता है, लेकिन इसमें समय लग सकता है और इस दौरान baby की close monitoring बहुत जरूरी होती है।

इस case में 39-year-old patient को irregular periods की problem थी, जिसके लिए diagnostic hysteroscopy और laparoscopy की गई। Hysteroscopy के दौरान uterus की cavity बिल्कुल healthy मिली—कोई fibroid, polyp या abnormality नहीं थी, और दोनों fallopian tubes भी patent थीं। A small biopsy भी confirmatory testing के लिए भेजी गई।

Laparoscopy में uterus का outer surface, tubes और ovaries को evaluate किया गया। Mildly enlarged ovaries और slight polycystic changes देखे गए, लेकिन overall reproductive anatomy healthy थी। Tubes में कोई blockage या hydrosalpinx नहीं था, जो IVF outcome के लिए positive sign है।

इस case में patient को ruptured ectopic pregnancy हुआ था, जिसमें pregnancy uterus के बाहर fallopian tube में develop होती है। जैसे-जैसे pregnancy बढ़ती है, tube फट सकती है (rupture), जिससे severe internal bleeding (hemoperitoneum) हो जाती है—जो life-threatening condition होती है।

Emergency laparoscopic surgery के दौरान right fallopian tube में मौजूद ectopic pregnancy को carefully remove किया गया। Suction और surgical instruments की मदद से pregnancy tissue को बाहर निकाला गया और bleeding को तुरंत control किया गया। Tube rupture होने की वजह से affected tube को भी remove करना पड़ा, ताकि patient की condition stabilize हो सके।

Down Syndrome in Pregnancy: कैसे Detect होता है? | Screening & Care Tips | Dr Mannan Gupta, Delhi

इस case में baby में multiple abnormalities देखी गईं—जैसे heart defect (VSD), kidney issues (hydronephrosis, single kidney), और limb abnormality (polydactyly)। इन signs के आधार पर Down syndrome का suspicion हुआ, जिसे amniocentesis से confirm किया गया।

ऐसे cases को early stage में detect करना बहुत जरूरी होता है। Pregnancy confirm होते ही doctor से consult करना चाहिए और timely scans करवाने चाहिए—जैसे 11–13+6 weeks पर dual marker test और NT scan, 18–22 weeks पर anomaly scan और fetal echo। ये tests baby में किसी भी defect को early identify करने में मदद करते हैं।
