Hyperkalemia 🚨⚡☢️

Serum potassium > 5.0 mEq/L
Causes (Remember: M A C K E R S)
M: Medications – ACEi, ARBs, K⁺-sparing diuretics, NSAIDs
A: Acidosis (metabolic)
C: Cell lysis – trauma, burns, rhabdomyolysis
K: Kidney failure → ↓ K⁺ excretion
E: Excess intake (rare alone)
R: Renin–aldosterone deficiency (Addison’s)
S: Shift of K⁺ from cells – insulin deficiency, beta-blockers
Clinical Features
Muscle weakness, paralysis
Paresthesias
Cardiac arrhythmias (most dangerous)
ECG: tall peaked T waves, wide QRS, sine-wave → arrest
Emergency Management
1. Cardiac protection:
IV Calcium gluconate
2. Shift K⁺ into cells:
Insulin + dextrose, β-agonists (nebulized salbutamol), Sodium bicarbonate (if acidosis)
3. Remove excess K⁺:
Loop diuretics, hemodialysis, sodium zirconium cyclosilicate / patiromer, Kayexalate (less used)
