In today’s IVF practice, transferring a Day 5 embryo (blastocyst) has become the standard approach in most cases.

Except for very rare situations—such as very low egg numbers or advanced maternal age—blastocyst transfer offers better outcomes compared to Day 2 or Day 3 embryo transfers.

Even though pregnancies can occur with Day 2 transfers, that does not make it the ideal or standard method. The reality is that not all embryos reach the blastocyst stage, and only the ones that do are more likely to result in a successful pregnancy.

If you are undergoing IVF, especially at a younger age, it is important to discuss and consider blastocyst transfer with your doctor for better chances of success.

IVF ट्रीटमेंट प्रत्येक पेशंटसाठी वेगळी असते. सगळ्यांना एकच approach वापरून success मिळत नाही.

काही cases मध्ये पुरुषाचा s***m count खूप कमी असतो, आणि repeated IVF cycles करूनही success मिळत नाही. अशा वेळी केवळ semen मधून s***m घेण्याऐवजी direct te**is मधून s***m घेतल्यास IVF success rate improve होऊ शकतो.

Fibroids are a very common cause of infertility, but not every fibroid needs to be removed. Only those fibroids which are large or pressing into the uterine cavity can affect your chances of pregnancy.

After fibroid removal, it is important to wait for around 3–4 months before planning further fertility treatment, so that the uterus heals properly.

Also, the way fibroids are removed and sutured plays a very important role. If the repair is not done properly, the scar can remain weak and may lead to complications during pregnancy.

That is why choosing the right center and experienced surgeons is extremely important when undergoing fibroid surgery for fertility.

Have you ever wondered how your embryo actually looked on the day of transfer?

What does zero s***m count actually mean?

It means there are no s***m present in the semen, a condition known as Azoos***mia.

Many patients immediately assume:
👉 “Now I will need a s***m donor.”

But that’s not always true ❌

👉 In nearly 50% of azoos***mia cases, it is still possible to achieve pregnancy using the patient’s own s***m.

How?

Through a specialized procedure called Micro-TESE (Microsurgical Testicular S***m Extraction), where s***m is directly searched and retrieved from the te**is using a microscope — even when none are seen in the ej*****te.

✅ So remember:
Zero s***m count does NOT mean zero hope.

Always consult an expert before making decisions about donor s***m.

This was one of the most difficult endometriosis cases we have encountered.

The patient had a 10 cm endometrioma on the left side along with extremely dense adhesions. During a previous laparoscopy attempt, even the uterus was not visible due to severe bowel adhesions.

In this surgery:
• Complete excision of the left-sided tubo-ovarian mass was performed
• The right o***y was carefully preserved
• Dense adhesions involving the re**um, ureters, and bladder were meticulously released
• Deep nodules and retroperitoneal extensions were completely removed

The patient is now endometriosis-free, and we hope she will be free from pain moving forward.

👉 Important Reminder:
Endometriosis surgery should always be performed at an experienced center.
Severe cases require expert surgeons and advanced techniques like 3D laparoscopy.

❗ Incomplete surgery can lead to persistent symptoms and recurrence.

Many couples think that once the egg is formed and ruptured, pregnancy is guaranteed… but that’s not true.

Ovulation Study has only 12–15% success rate per cycle.

Before starting ovulation tracking, it’s important to check:
• Fallopian tube status
• S***m count & quality
• Uterus condition
• Anti-Müllerian Hormone levels
• Overall findings on sonography

If everything is normal, then ovulation study can be continued for a few cycles with confidence.

But if:
• Endometrial lining is thin
• Infection is present
• Structural issues in uterus

Then simply doing ovulation study won’t help — proper diagnosis and treatment is needed.

👉 Remember:
Ovulation study is not for everyone. First check if you are eligible.

Infertility evaluation is not just about checking whether tubes are open.

Many patients come with a normal HSG (Hysterosalpingography) report showing open tubes… yet they are unable to conceive.

👉 The reason?
HSG cannot detect small but critical issues inside the uterus.

With Hysteroscopy, we can clearly see:
• Micro polyps
• Fibrin strands
• Fibrosis around fallopian tube openings
• Signs of hidden infection

These subtle findings are often missed on HSG but can directly impact fertility.

In such cases, simply doing ovulation studies or IUI for years won’t help.

✔️ First step is accurate diagnosis
✔️ Then treating infection with medications like Antibiotics
✔️ Followed by appropriate fertility treatment

Remember:
Open tubes do not guarantee pregnancy. Proper diagnosis does.

Today’s case is an interesting example of a unicornuate uterus, a condition where only one side of the uterus is fully developed.

In this procedure:
• We first performed a hysteroscopic metroplasty to improve the size and shape of the uterus
• After correction, the uterus appears more globular and capable of supporting pregnancy

👉 On evaluation:
• One fallopian tube is properly connected and open (confirmed with dye test)
• The opposite side shows a rudimentary horn, which has no connection to the main uterine cavity and does not contribute to fertility

✅ The good news:
• The o***y and functional tube are healthy
• Natural conception is still possible



Important points in Unicornuate Uterus:
1. Uterus Size Matters
If the uterus is small, procedures like metroplasty can help improve space for pregnancy
2. Cervical Incompetence
In some cases, a cervical cerclage (stitch) may be needed during pregnancy
3. Risk of Preterm Delivery
Proper counseling and monitoring are essential



💡 With the right treatment and approach, many patients with a unicornuate uterus can:
• Conceive naturally
• Carry a healthy pregnancy
