Let's listen to Dr Nikhil Bhangale,
Gastroenterologist on air, while he talks about Irritable Bowel Syndrome (IBS) on this world IBS awareness day (19 April 2025).

IBS is common chronic gastrointestinal
disease, characterised by either chronic or
recurring abdominal pain in combination with a change in bowel habits, such as constipation, diarrhoea or a mixture of both. IBS patients may have common symptoms like cramping, belly pain, abdominal fullness and gas. IBS affects 1 in 11 Indians at any point in time. Women are up to two times more likely than men to develop IBS

There are Few Factors which triggers IBS like Stress, depression, Gut microbiome, certain medication (such as antibiotics), certain foods (such as Processed food, carbonated drink, Alcohol, high protein diet), & Lifestyle habits
Three ways to manage the IBS

(1) Diet and nutritional management (Add fibres in diet, follow low FODMAP diet, eat five small meals every three or four hours)
(2) Behavioural management (get enough sleep
(3) Medical management

A middle aged lady (30 years) presented with chronic diarrhoea for almost 8 months with recently passage of blood in stool. She had visited many local clinics however no relief. So, we evaluated with stool anf blood tests. Due to very high suspicion, ileo-colonoscopy was done which showed ulcerative colitis (inflammation of large intestine) meaning lot of redness and ulcers as can be seen in above two images (before treatment). This patient was started on treatment for the same and she underwent repeat colonoscopy after 6 months (bottom two images) which shows complete resolution of inflammation seen earlier. Patient obviously improved symptomatically and overall helathwise as well dramatically post treatment.
So, it is very crucial to make a correct diagnosis for a correct and best treatment.
Beware of high risk symptoms like blood in vomiting or stool, always get yourself checked by best gastroenterologist near you.

FOREIGN BODY (COIN) REMOVAL

Case of a child 3 year old who accidentally swallowed a 1 rupee coin while playing with it, and presented with difficulty in swallowing as it was stuck just below throat, in upper esophagus as seen on xray chest and neck before procedure. So, patient was taken up for coin (foreign body ) removal endoscopically in OT. Coin was seen stuck in upper esophagus, difficult to grasp , hence it was pushed down gently with endoscope into stomach and then it was removed as shown in the image. Following procedure, patient was relieved of throat discomfort and was stable.
Just one rupee coin led to all this, so ironic.

Alcoholic liver disease (ALD)

It has spectrum of asymptomatic fatty liver, alcoholic steato-hepatitis, liver cirrhosis and severe alcoholic hepatitis (SAH).

Biopsy means cancer !! 😫

Truth: Biopsy only means taking sample from inside for examination, and it can be done endoscopically or sonography / CT scan guided. Biopsy is important because certain illness can be diagnosed from tissue structure like tuberculosis, IBD (inflammatory bowel diaease), H.Pylori infection, CMV infection, allergic conditions like eosinophilic enteritis, celiac disease, benign tumours and also of course cancers.

So, cancer is one of the reason to take, but not the only cause.
So next time, someone says biopsy, don't panic, just ask "why ??"

What is colorectal cancer screening ?

It is checking of large intestine by various modalities like colonoscopy, CT colonography, stool test like FOBT (F***l occult blood test) or FIT (f***l immunochemical test), stool DNA test.
These can help with early detection of cancer or precancerous lesions like polyps, which can be removed to prevent cancer.

Colonic polyps - precursors of colon cancer

Polyps are small growths which can involve any part of colon and are usually asymptomatic & benign (harmless), however they can be pre-cancerous. Evolution to cancer depends on various risk factors (modifiable & non- modifiable).
Removal of such polyps (colonoscopy with polypectomy) can reduce of future cancer development.
