The story of a 53 year old gentleman, severe chest pain & breathing problem, low BP (Cardiogenic shock) and low heart rate (complete heart block)- almost everything he had which can end his life in a few minutes. The right Coronary artery had 100% blockage (no blood flow in the right side of the heart at all). The left side had abnormalities in the origins of the artery along with 95% calcified blockage in the proximal LAD, 99% in the D1 ostium(which was a very large vessel). Angioplasty done successfully with 5 stents (both sides). Not for the first time, not the last time....but another story of a miracle (done by God obviously through my hands) in someone's life.... The patient went home walking & smiling. Time is the most important factor..... If you reach the right place early, there is always a chance of getting a new life... Me & my team are always ready to give our 200% to save a life, that's what we live for.... And again, God is great.

A 50 year old gentleman presented with chest pain and breathing difficulties. Coronary angiography - Multiple blockage in all three major arteries and one of them have 100% blockage. What do you do next? Bypass surgery would be advised by many. After discussing with the patient relatives, I successfully opened 6 blockages in 1.5 hours... The patient is smiling while walking out of the Cathlab. Absolutely normal flow in all three major arteries after operation ( like a normal coronary artery). Who needs Bypass surgery when you have a Cardiology team like us nearby you !! The days are gone when more than 2 blockages were advised for CABG. 6 Stents were put in this patient and of course he is not my first patient to do so, many of them are living normal life after Angioplasty with multiple blockages. Again, God is great and ultimately he is the one who makes every impossible things possible.... 😊

Thriller:

This 50 year gentleman was found to have a complex LMCA Bifurcation lesion (Medina 1,1,1) which was planned for bifurcation angioplasty using the DK Crush technique. After the first kissing balloon inflation (KBI) done the patient became unstable hemodynamically and also developed severe chest pain. In that situation where it's natural to panic for a young cardiologist, It was difficult to continue DK Crush as I was running out of time. The provisional stenting technique wasn't a good idea because I could lose LCX anytime which could make the situation worse. Then what I did was to take 2 stents inflating & kissing together ( I have never seen anyone doing that before) to make sure that both LAD and LCX were flowing nicely. The patient became stable and I could go with the 2nd kissing balloon inflation and POT. The patient is absolutely fine and discharged from the hospital with no more angina. Hoping for a long term good outcome of kissing with 2 stents technique in LMCA complex Bifurcation lesions.... LMCA Angioplasty is what gives me thrill and satisfaction. I thank my patient, my team and the Almighty always 🙏

Magic in Cardiology:

A 23 year old girl who was very sick looking during admission, presented with BP 250/140 mmHg.....which was not responding to any treatment, could not be controlled even after taking 5 drugs for hypertension. After the workup, I found that both the renal artery (Blood supply of both Kidney) is severely stenosed. Today, I successfully performed Angioplasty of both renal arteries after which BP came down to 150/100 mmHg like magic. One of the rarest cases definitely, but very very rewarding for the patient. Happy patient Happy doctor.... And congratulations to my team for performing this very rare case, first time in Durgapur....... 😎

Another day.... Just another Angioplasty...but this time it's LMCA Bifurcation Angioplasty.... No second thought will be given other than Bypass surgery (CABG) in most centres for such lesions.... But we did it, DK Crush technique it is..... Congratulations to my team 😁😎... And I am always thankful to the patients who believe in me and my team for performing such complex intervention.... And last but not the least, God is always kind 🙏

Another milestone.... Cardiac Resynchronization Therapy (CRT) implanted in a female with Ejection fraction (EF)- 22%....... Significant improvement in QRS duration on the table itself (190ms pre-CRT to 120 ms post CRT).... Wishing you all a happy Durga Puja.....

Another day, Another patient... but the thrill for LMCA Angioplasty never dies... The patient refused to undergo Bypass surgery, and I opened it without cutting and opening the chest.. Happy World Heart Day .... "Use ❤️, Know ❤️"..... Happy patient, Happy Doctor 😀

Left main coronary artery Angioplasty, one of the most dangerous and most difficult Angioplasty is definitely an alternative to Bypass surgery..... This patient is a 43 year old lady with diabetes and high cholesterol presented with a deadly heart attack, low BP, difficulty in breathing... so called Cardiogenic shock ( 50% of such patients die even in the best of the centres).... The ejection fraction was just 20% (Normal around 60%)...... After such a difficult and life saving Angioplasty patient improves so much drastically, it feels good to be an Interventional Cardiologist....... Thanks to my patients who always keep faith in me for such complicated procedure......

Thrill/Satisfaction/Cardiology:

75 year old lady with low hemoglobin and kidney disease, presented chest pain and difficulty in breathing since one year, altered consciousness for 1 week because of the complete heart block of the (heart was beating at 30/minutes which is incompatible with life)... temporary pacemaker maker was inserted to stabilize initially. Angiography was difficult from hand (radial approach) because of arteria lusoria, however I did with the guide catheter with difficulty. Angiography revealed 100% block of coronary artery with colleterals (Chronic total occlusion)..... The patient was very restless, even after sedation 5 people held her throughout the angioplasty, 3 times she has thrown out the whole angioplasty system from the coronary artery...... Finally after all the drama inside cathlab, completed the angioplasty successfully with TIMI III flow....best part was her own heart rhythm came back immediately and we were able to remove the temporary pacemaker too...... She is doing well now...... Life isn't easy, but it's beautiful....
