কোলোরেক্টাল ক্যান্সার এমন একটি রোগ যা প্রতিরোধযোগ্য, কিন্তু সচেতনতার অভাবে অনেক সময় দেরিতে ধরা পড়ে। এই ভিডিওর মাধ্যমে আমি দেখানোর চেষ্টা করেছি কীভাবে কোলনের বিভিন্ন অংশে, অ্যাসেন্ডিং, ট্রান্সভার্স, ডিসেন্ডিং কোলন এবং রেকটামে, ক্যান্সার তৈরি হতে পারে।

🔍 প্রাথমিক পর্যায়ে শনাক্তকরণ জীবন বাঁচাতে পারে।
অনেক ক্ষেত্রে ছোট ছোট পলিপ (growth) থেকে এই রোগ শুরু হয়, যেগুলো শুরুতে কোনো লক্ষণ দেখায় না কিন্তু সময়ের সাথে ক্যান্সারে পরিণত হতে পারে।

Rare Duodenal Tumor Successfully Treated with Robotic Surgery

A man in his early 40s was successfully treated for a rare duodenal tumor after presenting with persistent dyspepsia and intermittent episodes of melena for six months.
Further evaluation with upper gastrointestinal endoscopy revealed a large, broad-based submucosal lesion arising from the first part of the duodenum and extending into the second part. The patient underwent robotic-assisted segmental duodenectomy with loop gastrojejunostomy using the da Vinci Xi system. There was negligible blood loss.
Histopathological examination later confirmed Brunner’s gland hamartoma, an extremely rare benign tumor of the duodenum. The patient recovered uneventfully, resumed oral intake within days, and was discharged in stable condition.

চল্লিশের কোঠায় এক ব্যক্তির বিরল ডুওডেনাল টিউমারের সফল চিকিৎসা করা হয়েছে রোবোটিক সার্জারির মাধ্যমে। রোগী প্রায় ছয় মাস ধরে দীর্ঘদিনের হজমের সমস্যা (ডিসপেপসিয়া) এবং মাঝে মাঝে মেলেনা বা কালো পায়খানার সমস্যায় ভুগছিলেন।
পরবর্তীতে উপরের জিআই এন্ডোস্কপি করার পর দেখা যায় যে ডুওডেনামের প্রথম অংশ থেকে উৎপন্ন হয়ে দ্বিতীয় অংশ পর্যন্ত বিস্তৃত একটি বড় ও চওড়া ভিত্তিযুক্ত সাবমিউকোসাল লেশন রয়েছে। রোগীর ক্ষেত্রে da Vinci Xi রোবোটিক সিস্টেম ব্যবহার করে রোবোটিক-সহায়তায় সেগমেন্টাল ডুওডেনেকটমি এবং লুপ গ্যাস্ট্রোজেজুনোস্টমি করা হয়। অপারেশনের সময় রক্তক্ষয় খুবই কম ছিল।
অপারেশনের পর টিস্যুর হিস্টোপ্যাথোলজিক্যাল পরীক্ষায় নিশ্চিত হয় যে এটি ব্রুনার্স গ্ল্যান্ড হ্যামার্টোমা, যা ডুওডেনামের একটি অত্যন্ত বিরল ও অম্যালিগন্যান্ট টিউমার।
অপারেশনের পর রোগীর সুস্থতা দ্রুত হয়, কয়েক দিনের মধ্যেই তিনি স্বাভাবিকভাবে খাবার গ্রহণ শুরু করেন এবং স্থিতিশীল অবস্থায় হাসপাতাল থেকে ছুটি পান।

How I Do It: Robot-Assisted Segmental Duodenectomy for Giant Brunner's Gland Hamartoma

Brunner’s gland hamartoma (BGH) is a rare benign tumor of the duodenum, most commonly arising from the first and second parts. Although often asymptomatic, large lesions may present with dyspepsia, anemia, gastrointestinal bleeding, or obstruction. We describe a man in his early 40s with dyspepsia and intermittent melena for six months. Endoscopy revealed a large broad-based submucosal mass extending from the first to the second part of the duodenum. Contrast-enhanced CT demonstrated a well-circumscribed 7 cm lesion in the post-pyloric region, initially suspected to be a gastrointestinal stromal tumor.

The patient underwent robotic-assisted segmental duodenectomy with loop gastrojejunostomy. A 6.5 × 4 cm cylindrical polypoidal mass was resected. Histopathology confirmed Brunner’s gland hamartoma. Postoperative recovery was smooth, with early oral intake and discharge on day four. The patient remained symptom-free at follow-up, demonstrating the effectiveness of robotic resection for large duodenal hamartomas.

ব্রুনারের গ্রন্থির হ্যামারটোমা (BGH) হলো ডুওডেনামের একটি বিরল সুষম (benign) টিউমার, যা সাধারণত প্রথম ও দ্বিতীয় অংশে দেখা যায়। বেশিরভাগ ক্ষেত্রে উপসর্গহীন হলেও বড় আকারের টিউমার ডিসপেপসিয়া, রক্তাল্পতা, গ্যাস্ট্রোইনটেস্টাইনাল রক্তক্ষরণ বা অবরোধের কারণ হতে পারে। আমরা চল্লিশের কোঠার এক পুরুষ রোগীর কথা উপস্থাপন করছি, যিনি ছয় মাস ধরে ডিসপেপসিয়া ও মাঝে মাঝে মেলেনায় ভুগছিলেন। এন্ডোস্কোপিতে প্রথম থেকে দ্বিতীয় অংশ পর্যন্ত বিস্তৃত একটি বড়, ব্রড-বেসড সাবমিউকোসাল মাস ধরা পড়ে। কনট্রাস্ট-এনহ্যান্সড সিটি স্ক্যানে পোস্ট-পাইলোরিক অঞ্চলে সুস্পষ্ট সীমানাযুক্ত ৭ সেমি লেশন দেখা যায়, যা প্রাথমিকভাবে জিস্ট (GIST) সন্দেহ করা হয়।

রোগীর রোবোটিক সহায়তায় সেগমেন্টাল ডুওডেনেক্টমি ও লুপ গ্যাস্ট্রোজেজুনোস্টোমি করা হয়। ৬.৫ × ৪ সেমি সিলিন্ড্রিক্যাল পলিপয়েডাল মাস অপসারণ করা হয়। হিস্টোপ্যাথোলজি ব্রুনারের গ্রন্থির হ্যামারটোমা নিশ্চিত করে। অপারেশনের পর রোগীর দ্রুত সুস্থতা হয় এবং চতুর্থ দিনে ছুটি দেওয়া হয়। ফলো-আপে রোগী সম্পূর্ণ উপসর্গমুক্ত ছিলেন।

It was a fantastic academic evening yesterday!
A non-stop 2.5-hour CME was held on the role of robotics in Gastrointestinal and Hepato-Pancreato-Biliary (GI & HPB) surgery, along with a video demonstration of our robotic and laparoscopic procedures. We had an engaging discussion on techniques and technology used in complex biliary and pancreatic surgeries.

We were honored by the presence of Dr. Biswanath Mukherjee, Senior General & Laparoscopic Surgeon from Bankura, and Dr. Debabrata Ray, Head of the Department of General Surgery, Bankura Sammilani Medical College & Hospital.

The session concluded with an excellent interactive discussion and very encouraging feedback from colleagues with a request to organize another seminar within the next 2–3 months.

Grateful for the enthusiastic participation and academic spirit of everyone involved. Looking forward to the next session!

Happy to share that our video on robotic work, “Robotic-assisted Choledochal Cyst Excision with Hepaticojejunostomy,” has been selected as the First Prize Winner under the ABSICON 2021 Young Surgeon Award category at SASICON 2026 (the West Bengal Chapter of the Association of Surgeons of India), held on 14–15 February.

It was a very well-conducted annual conference organized by the Society of Bengal Surgeons.

We hope to continue contributing new innovations and technology in the service of the people of Bengal and to further enrich the pride of our state in the future.
