Title :- Comprehensive Management of Multiple Orofacial Lacerations Following a Road Traffic Accident: A Case Report

Abstract:
This case report presents the successful surgical management of multiple facial and intraoral soft tissue injuries in a patient following a road traffic accident. Prompt evaluation and meticulous suturing led to effective wound closure and favorable healing outcomes. This report emphasizes the importance of timely and organized approach in handling complex orofacial trauma cases.

Introduction:
Facial soft tissue injuries are common in road traffic accidents and may involve multiple anatomical regions including the lips, tongue, nose, and ears. Such injuries require a multidisciplinary approach and timely intervention to prevent long-term functional and aesthetic complications.

Case Report:
A patient presented to our clinic following a recent road traffic accident with multiple soft tissue injuries. On clinical examination, the patient was found to have:

A deep laceration on the forehead

A deep cut involving the right nostril

A deep laceration of the right ear

A deep cut on the tongue

A laceration on the lower lip

All wounds were thoroughly cleaned under aseptic conditions. Hemostasis was achieved, and layered closure was performed using appropriate suture materials. The patient was prescribed antibiotics and analgesics, and advised on post-operative wound care and follow-up.
Discussion:
Multiple facial injuries in a single trauma case pose challenges in wound closure, tissue approximation, and infection prevention. In this case, careful assessment and prioritization of wounds allowed for successful management. Suturing of the tongue and lip required special attention to preserve function and avoid scar-related issues.

Conclusion:

Clinical and radiographic evaluation revealed a horizontally impacted mandibular third molar (tooth 48) and a pulp polyp in the adjacent second molar (tooth 47). Surgical removal of the impacted tooth was carried out using a sectioning technique under local anesthesia, followed by proper irrigation and suturing. The patient was stable post-operatively and advised appropriate follow-up. This case emphasizes the importance of comprehensive diagnosis and prompt surgical intervention in managing impacted third molars and associated lesions.
Introduction:
Impacted mandibular third molars are frequently encountered in dental practice. Their impaction may lead to various complications, including pericoronitis, caries, resorption of adjacent teeth, or pulpal pathologies. Pulp polyps, or chronic hyperplastic pulpitis, often arise due to long-standing carious exposure. Timely surgical intervention is crucial to prevent further complications.

Case Report:
A patient reported to our clinic with the chief complaint of pain in the lower right jaw region. On clinical examination, grossly decayed tooth 47 was noted. To evaluate the underlying cause and the condition of adjacent teeth, an orthopantomogram (OPG) was advised.

Radiographic analysis revealed a horizontally impacted mandibular third molar (tooth 48) and a pulp polyp associated with tooth 47.

Surgical extraction of the impacted 48 was planned. Under local anesthesia, a full-thickness mucoperiosteal flap was raised. The crown and roots were sectioned using a surgical handpiece, and the tooth was carefully removed in parts. Copious irrigation with saline was done, and the surgical site was sutured with 3-0 silk sutures. Postoperative instructions and medications were given.
Discussion:
Horizontal impaction of third molars poses significant surgical challenges and often contributes to the pathology of adjacent molars. In this case, the impacted 48 likely contributed to the development of pulp polyp in 47 due to carious invasion or difficulty in maintaining hygiene. Gingivitis is a common periodontal condition, often characterized by inflammation, redness, and bleeding of the gingiva. However, in certain cases, it may present as painless swelling, which can delay treatment. This report presents a case of localized gingivitis with unusual presentation, managed through surgical intervention.

Case Presentation:
A patient reported to our clinic with a complaint of unusual swelling in the gingiva, which had been present for around two months. The swelling was localized and non-tender, with no associated pain or bleeding.

Clinical Findings:

A pocket marker was used to outline the affected gingiva.
