FIELD VISIT

Date of Visit: 21.05.2025
Location: Lower Kokaley Area.
A field visit was conducted in the Lower Kokaley area under the jurisdiction of Makha HWCSC. Despite difficult and rugged terrain, the team successfully reached the home of a palliative and cancer patient to deliver essential healthcare services. The visit aimed to ensure continuity of care for vulnerable individuals with limited access to institutional health services.
Visiting Team:
• MLHP (Mid-Level Health Provider)
• ANM (Auxiliary Nurse Midwife)
• ASHA (Accredited Social Health Activist)
• Female Health Attendant

Activities Carried Out:
1. Palliative Care:
Provided comfort-focused palliative care and counseling to a cancer (CA) patient to improve quality of life.
2. Geriatric Care:
Delivered health assessment, basic treatment, and wellness advice for elderly individuals requiring geriatric support.
3. Postnatal Care (PNC):
Conducted a home visit to monitor the health of a new mother and her newborn, providing essential postnatal guidance.
4. NCD Management:
Delivered home-based follow-up and counseling for patients living with Non-Communicable Diseases (NCDs), including monitoring of vital signs and medication adherence.

Observations & Recommendations:
• Continued follow-up is recommended for the palliative care patient.
• Improved transportation or community-based support may be necessary to reach such difficult terrain areas more efficiently.
• Community awareness on home-based care services should be strengthened.
• NCD patients benefit significantly from routine home visits—scale-up may be considered

🩺 NCD cm VHND Conducted on World Hypertension Day 2025

📍 Venue: Lwr Singbel, under Makha HWCSC

On the occasion of World Hypertension Day, an NCD cm VHND (Non-Communicable Disease cm Village Health & Nutrition Day) session was successfully conducted at Lwr Singbel to raise awareness on the importance of blood pressure management and cardiovascular health. The event also included palliative home-based care services for patients in need.

NCD Screening, VHND, and Immunisation Week Celebration
Venue: Paksam ICDS Centre

On 25th April 2025, a comprehensive outreach programme was conducted at Paksam ICDS Centre as part of Village Health and Nutrition Day (VHND), in alignment with the ongoing Immunisation Week celebration. The programme also integrated Non-Communicable Disease (NCD) screening and awareness, with the objective of improving preventive health practices among the community.

Key Activities Conducted:
• Screening for NCDs such as hypertension and diabetes (BP & blood sugar )
• Growth monitoring of under-5 children
• Distribution of IFA, calcium, and deworming tablets
• Counselling sessions on immunisation, nutrition, hygiene, and prevention of lifestyle diseases

Health Education Topics Included:
• Importance of complete immunisation
• Early detection and management of NCDs
• Balanced diet and lifestyle modifications
• Handwashing and hygiene promotion

Awareness Programme at Children Preparatory School

Topic: Chickenpox, Diarrhoea & Vomiting
Venue: Children Preparatory School
Date: 24/04/2025

An awareness programme was conducted at Children Preparatory School on 24th April 2025 to educate students about common health issues such as diarrhea, vomiting, and chickenpox. The session aimed to promote good hygiene practices and increase awareness about early symptom recognition and preventive care.

The programme was organized by CP School in coordination with Makha HWCSC, with active participation from:
• Staff of Makha HWCSC
• Principal of CP School
• School staff and parents

Key Focus Areas:
• Causes, symptoms, and home care for diarrhea and vomiting
• Understanding chickenpox: transmission, symptoms, and prevention
• Importance of hand hygiene, clean drinking water, and personal cleanliness
• Demonstration of ORS preparation and handwashing techniques

The session concluded with a Q&A segment and interactive discussion involving students and parents, ensuring better understanding and engagement.

The programme was well-received and appreciated by all present, reinforcing the importance of preventive health education at the community level.
