Sharing an interesting case i managed recently.
A newborn male baby presented with excessive weight loss , lethargy and poor feeding on day 24 of life .
Investigation revealed negative sepsis screen , normal blood sugar and severe hyponatremia (Na 115) and no acidosis.
Baby was managed as a case of Salt Wasting type Congenital Adrenal Hypeplasisia.
Diagnosis was later confirmed by raised levels of 17 OH Progesterone and Testosterone.
Baby recovered fully and is doing well now.
Take home message: Newborn screening for all is mandatory, and dont forget noninfectioius diseases as a cause of lethargy .
