✅ Exercise therapy for flatten foot

1- Performing dorsiflexion and inversion movements in the ankle while keeping the toes in flexion.

2- By keeping the limbs in the extension position next to each other, the patient should try to bring the soles of the feet together.

In sitting position:

3- Place the left limb on the right limb and do dorsiflexion, inversion and plantar flexion in the left leg in a semicircle, and then rest (do not do aversion). Repeat this exercise for the right leg.

4 - While the knees are apart, keep the soles of both feet in contact with each other and then bring the two knees together.

5- Place a towel on the ground and place the soles of both feet on it with a distance of about 15 cm from each other, and then the patient grabs the towel with his fingers and pulls it in (with adduction) and the towel between the two feet. Collect.

6- Place a small ball under the front arch of the foot and catch the ball with your fingers.

Standing:

7- While the legs are forward or slightly toe-out, transfer the weight of the body to the outer edge of the foot by raising the arches.

In walking position:

8- While the feet are forward, walk on the ground along a straight line and transfer the weight of the body from the heel to the outer edge of the foot and then to the toes.

✅ Extensor Tendon-Gliding Exercises

Diffrential gliding of the extensor digitorum communis tendons to each of the fingers can be achieved by the following progression.

- Teach the patient to passively flex the MCP and IP joints of one finger with the opposite hand while actively maintaining the other fingers in extension.
- If the patient has difficulty doing this, begin with the involved hand resting on a table with the palm up. Stabilize three of the four fingers against the table while passively flexing one of the digits. Then instruct the patient to attempt to actively keep the fingers against the table while one of the digits is passively flexed.
- Progress by having the patient actively maintain the fingers in extension with the fingers spread out and then actively flex each finger in turn while the other fingers remain extended.
- Have the patient flex the middle and ring fingers while maintaining extension of the index and little fingers (long horn sign). This promotes isolated control of the extensor indicis and extensor digiti minimi tendons and promotes their gliding on the extensor digitorum communis tendons.

Carpel Tunnel syndrome

Physiotherapy :

Physiotherapy is beneficial for most carpal tunnel sufferers especially in mild to moderate cases.

1️⃣Carpal bone mobilisation and flexor retinaculum stretching to open the carpal tunnel. Tal-Akabi & Rushton (2000).

2️⃣Nerve and tendon gliding exercises to ensure full unrestricted nerve motion is available. McKeon & Hsieh (2008).

3️⃣Muscle and soft tissue extensibility. Moraska (2008).

4️⃣Cervicothoraic spine to correct any referral or double crush syndromes. Kwon et al (2006).

5️⃣Grip and pinch, thumb abduction and forearm strengthening in later phases. Pinar et al (2005).

6️⃣Comprehensive upper limb, wrist and hand ROM strengthening and endurance exercises.
Posture, fine motor and hand dexterity exercises. Abd-Elkader et al (2010).

7️⃣Ultrasound Therapy, Acupuncture:

➡️Ultrasound therapy has been shown to assist carpal tunnel syndrome. Huisstede et al (2010). Acupuncture has been shown to decrease carpal tunnel pain. Sim et al (2011), Kumnerddee & Kaewtong (2010), Yang et al (2011).

8️⃣TENS Machine:

➡️A TENS machine (transcutaneous electrical muscle stimulation) has been shown to ease the pain associated with carpal tunnel syndrome. Kara et al (2010)

9️⃣Bracing or splinting:

➡️Wearing a brace or splint at night will keep you from bending your wrist while you sleep. Keeping your wrist in a straight or neutral position reduces pressure on the nerve in the carpal tunnel. It may also help to wear a splint during the day when doing activities that aggravate your symptoms.

🔟Nerve gliding exercises:

➡️Some patients may benefit from exercises that help the median nerve move more freely within the confines of the carpal tunnel. Specific exercises may be recommended by your doctor or therapist.

Carpel Tunnel Syndrome

♿Phalen’s Test:

🛐Purpose:

To test for carpal tunnel syndrome (CTS)

👨‍⚕️Technique:

➡️The patient places her flexed elbows on a table, allowing her wrists to fall into maximum flexion. The patient is asked to push the dorsal surface of her hands together and hold this position for 30-60 seconds.

➡️This position will increase the pressure in the carpal tunnel, in effect compressing the median nerve between the transverse carpal ligament and the anterior border of the distal end of the radius.

➡️Another way to describe the standard Phalen's test: the patient leaves his / her elbows bent between 0 ° -30 °. Then the therapist asks the patient to supinate his / her forearm.

➡️After this the therapist carries a maximum palmar flexion of the wrist and holds it for 60 seconds. During the test the patient will be asked to explain each 15 seconds what she feels.

⚛️Positive test:

Phalen test are considered positive when the patient's symptoms are reproduced, test gives the same symptoms as that experienced with carpal tunnel syndrome, namely paresthesia (buring, tingling, numbness) in the distribution of the median nerve.

⚛️Negative test:

Patient feels no pain or any other symptom, even if the motion is maintained for three minutes or longer.

Osteoarthritis Knee

👨‍⚕️Dignose:

The diagnosis can be established by clinical examination, and it can be confirmed by X-rays.

✴️Knee OA can be sub-divided into 5 grades:

1️⃣Grade 0:

This is the “normal” knee health.

2️⃣Grade 1:
Very minor bone spur growth and is not experiencing any pain or discomfort.

3️⃣Grade 2:

This is the stage where people will experience symptoms for the first time. They will have pain after a long day of walking and will sense a greater stiffness in the joint. It is a mild stage of the condition, but X-rays will already reveal greater bone spur growth. The cartilage will likely remain at a healthy size.

4️⃣Grade 3:

Moderate OA. Frequent pain during movement, joint stiffness will also be more present, especially after sitting for long periods and in the morning. The cartilage between the bones shows obvious damage, and the space between the bones is getting smaller.

5️⃣Grade 4: This is the most severe stage of OA. The joint space between the bones will be dramatically reduced, the cartilage will almost be completely gone and the synovial fluid will be decreased. This stage is normally associated with high levels pain and discomfort during walking or moving the joint.
