Ruptured ACOM aneurysm which has recurred after clipping. The aneurysm had a wide neck and we performed stent-assisted coiling with “shelfing technique” (seen in the video as to how we expanded the stent to cover the whole of the neck). We also repositioned the catheter using wire torque to get adequate coiling (seen in the second video). At the end achieved adequate coiling. The case shows that even after clipping aneurysms can recur and we need long term to follow up.

Young male patient with rapid deterioration due to intramedullary spinal AVM at D12-L1 level. The feeders were only from the anterior spinal axis. We catheterised the sulco-commissural branch arising from ASA and did selective glue embolization. Microcatheter injection showed Intra nidal aneurysms. Post embo angio showed a small residual nidus with preserved anterior spinal axis (arrows). The patient did well. Anterior spinal artery embolization can be done by the analysis of angioarchitecture and selective catheterisation - embolization.

Challenging case of severe carotid stenosis with irregular plaque. There was a kink in the artery just distal to the narrowing which made it difficult to place the filter device. We navigated a neuro wire across the stenosis which straightened the artery, thereafter we could take the filter across. Stenting was uneventful and the filter device trapped embolic material. In such complex plaques, we feel filter device adds to the safety and we should make all attempts to use it.
