Cobb Angle — The Clinical Language of Scoliosis

Scoliosis is not just about how the spine looks — it’s about how we measure it.

The Cobb angle is the gold standard used worldwide to quantify spinal curvature on an X-ray.

But here’s where most people get it wrong:
It’s not about randomly drawing lines — it’s about selecting the correct vertebrae.

• The most tilted vertebra at the top of the curve
• The most tilted vertebra at the bottom
• Lines along their endplates
• Perpendiculars → angle measurement

That final angle defines severity and guides treatment decisions.

Clinical interpretation matters more than just measurement.

Because in practice — we don’t treat angles…
We treat people.

In a recent podcast, I was asked about the most common physical problems people are facing today. From my clinical experience, three issues clearly stand out: back pain, neck pain, and shoulder pain.
Interestingly, the age group of patients has changed dramatically over the years. Earlier, most people with spine-related problems were around 60-65 years old, mainly due to age-related degeneration.
With the computer era, prolonged sitting and poor posture started affecting people in their 40s. Now in 2026, we regularly see patients in their 30s suffering from chronic neck and back pain due to excessive screen time, mobile use, and sedentary lifestyles.
Another major factor today is stress, which has increased significantly after the COVID period. Stress often leads to muscle tension, poor sleep, and increased pain sensitivity.
What is even more concerning is that young children are also starting to develop neck and back discomfort. With both parents working, kids often spend long hours on mobile phones and tablets, even though their spinal curves are still developing.
If this trend continues, we may see even more spinal problems in the next generation.
Understanding this shift is important-because modern pain is no longer just about aging. It is about lifestyle, posture, stress, and technology.

What is Pitting Edema?
Pitting edema is swelling in which pressure leaves a temporary indentation ("pit") in the skin, indicating excess interstitial fluid. It is commonly seen in lower limbs (ankle, shin, foot).
What Does It Indicate?
Pitting edema is usually associated with fluid overload or poor circulation:
Common Causes
• Cardiac → Congestive heart failure
• Renal → Kidney disease, nephrotic syndrome
• Hepatic → Liver cirrhosis, low albumin
• Venous insufficiency
• Prolonged standing / immobility
As a physiotherapist & osteopath, pitting edema is not just swelling —it reflects systemic and fluid dynamics dysfunction:
• Fascial restriction can impair lymphatic flow
• Diaphragmatic restriction → reduces venous & lymph return
• Visceral congestion (liver, kidneys) → affects fluid regulation
• Autonomic imbalance → alters vascular tone.
Important Differentiation
• Pitting edema → fluid overload (leaves indentation)
• Non-pitting edema → lymphatic or fibrotic (e.g., lymphedema, myxedema)

That’s why training like this helps:

✔ Enhance joint stability
✔ Build functional strength

This is not just exercise…
