🦷 Surgical extraction of an impacted third molar with severe root curvature in close proximity to the inferior alveolar nerve 🚨. Pre-operative assessment revealed a dilacerated root directly adjacent to the inferior alveolar canal 📍. Patient was advised for a CBCT scan 📸 but patient was not willing and refused 🙅‍♂️. Although consent was taken from the patient 📝.

Conservative surgical approach was chosen to protect the nerve 💡: crown sectioning followed by careful root removal ensured a safe and predictable outcome 🌟, with no postoperative complications 🙏. Precision 💪, planning , and respect for the anatomy 💯 are key in such cases 🦷.

•ANOTHER CASE OF FILE RETRIEVAL

•A patient was told that her lower right premolar and molar teeth should be extracted( after performing the root canal by some nearby dentist), but she was unable to decide and insisted on having a root canal.
•She came to our clinic with complain of pain since 15-20 days.
Radiograph was made and revealed a broken file in mesial canal of first molar.
•I tried to retrieve the file but nothing happened.
•After spending almost 30 mins I decided to by pass the file.
Finally the 15no. K file bypassed but the broken file shifted apically (little bit).
•I tried and tried and finally a rotary file (S2) bypassed.
•A last attempt was made to retrieve the file and finally succeed.
After so much effort, the fragment was eventually recovered.

• Story of a maxillary first molar that turned into the nightmare.
• A new patient was came to our clinic with complain of pain in upper right back tooth region.
•I did an IOPA of the tooth , IOPA reveals- caries was approaching the pulp.
I told the patient to go for rct.
• But it turned into a nightmare when a shaping file broke in disto buccal canal.
• I got complete blank what to do next with so much patient trust on me.
• Recalled the patient for next visit started again with calm and free mind with complete patience finally able to bypass it after an hour.
• Finally relieved and further prepared it till F3
•The only key for bypass is patience, patience and patience.
