Last year, I met Professor Stephano Bini, Professor of Orthopedic Surgery at the University of California San Francisco, as he traveled around the world to better understand global healthcare’s transformation. I met him in Bordeaux, France, where we connected over our shared passion for Personalised Knee Arthroplasty TKA.

That evening in Mumbai, we chatted about my journey in establishing a hospital in India’s heartland and the challenges of bringing world-class technology to tier-III cities like ours.

One of the most complex cases to manage in knee replacement is Valgus deformity.
The question often asked is whether the medial envelope stretched and the lateral side contracted.
This is a false notion because ligaments cannot stretch or contract( they can only break)
With the kinematic alignment technique, it’s easy to balance such cases. Before surgery, one should ask if the patient was neutral/ varus before or was Valgus-oriented all his life.
In this case, ROSA robotic was used to balance soft tissue and achieve a final Valgus alignment of 1.5 degrees without any soft tissue release.
Note: The nail head was within the cuts, so we still had to remove it.

The first personalized arthroplasty meeting of its kind in Pune was a resounding success, setting the stage for further advancements in the field. The event attracted a diverse group of orthopedic surgeons, whose thoughtful questions and eagerness to learn fueled vibrant discussions. This level of engagement not only enriched the participants but also provided us, as educators, with valuable feedback on how to enhance future meetings.

Looking ahead, we are excited to take the next step by creating a data collection platform. This platform will enable more systematic tracking of personalized arthroplasty outcomes, providing a solid foundation for evidence-based improvements in patient care. Furthermore, we are exploring the possibility of establishing a national registry for Kinematic Aligned Total Knee Arthroplasty (TKA). Such a registry would be an invaluable resource for the entire medical community, facilitating research and ensuring that the highest standards of care are consistently maintained across the country.

This event marks the beginning of a concerted effort to push the boundaries of personalized arthroplasty, with a commitment to continuous improvement and collaboration. We are eager to see how these initiatives will shape the future of the field.
