Gastroenteritis #

* With Monsoons approaching Gastroenteritis ( loose motions and vomiting ) will be more prevalent

* This is due to consumption of contaminated water and food

* Dehydration due to loss of water and electrolytes is the main complication.

* lethargy, decreased oral intake and decreased urine output are the symptoms of dehydration

* To prevent this child should be given plenty of fluids like coconut water, buttermilk,lemonade and ORS ( WHO certified).

* Cleaning hands with soap after play and toilet use, drinking boiled and filtered water, avoiding consumption of street food…..are some preventative measures.

NOSE BLEED ( EPISTAXIS ) is more common in Summers due to dryness of nasal mucosa as a result of low humidity.
Most common cause being Nose picking by children f/by injuries, nasal polyps, Deviated nasal septum, bleeding disorders, etc
Treatment- do as shown in pic and you can apply ice packs or cold compresses.
Recurrent nosebleeds irrespective of seasons needs evaluation. Consult a Paediatric / ENT spl

Common infection during summer *CHICKEN POX*
- A contagious viral infection affecting skin which is vaccine preventable
- Fever, mild sore throat precedes onset of skin rash, itching
- Rash starts on abdomen and back …. Spreads to limbs and face
- It spreads by droplets and direct contact. Keep your child in isolation for 7 days
- Treatment is symptomatic…. Paracetamol for fever, anti allergic medication and calamine lotion

- Watch for Danger signs:
High grade fever with
1. Cough ➡️ Pneumonia
2. Headache, vomitings ➡️ Meningitis
3. Pain and swelling in sc***um ➡️ Orchitis

- Chicken pox vaccination efficacy is 60-70%…. Your child may get it even after vaccine is given but it prevents complications like pneumonia, orchitis, memingitis.

Please consult a doctor and don’t neglect your child’s health in the name of god or evil without giving medication…..

*SUMMER TIPS ( HYDRATION )*

*For Newborns and Infants *
* Upto 6months of age only Breast milk is sufficient NO WATER to be given
* 6months to 1 yr babies can be given sips of water every 2-3hrs

*For toddlers and children*
* Hydration can be maintained with water, fresh fruit juices, buttermilk, lemonade, coconut water
* NO SOFT DRINKS OR PACKAGED FRUIT JUICES to be given

*Persistent and recurrent cold and cough & Recurrent ear infections in babies*

Are you feeding your baby with a bottle, especially when baby is lying flat or sleeping?

If so, please do away with the bottle and stop sleep feeding / feeding the baby when he/she is lying flat. You will see a dramatic improvement.

Consider other causes only after you have eliminated this factor.

*Bottle feeding is supposed to be the single most important factor responsible for recurrent ear infections*

*Foreign bodies*
coins, marbles, pebbles, capsules, magnets, button batteries, and the list goes on.

Most of these swallowed foreign bodies will harmlessly pass through the gut without any complications, and come out in the stools. Some of them can prove to be risky.

The risky ones are
1. Very large foreign bodies
2. Very small ones which can stick to the gut
3. Sharp objects like pins and razors
4. Objects like button batteries which can cause burns
5. Magnets and Iron objects, which can erode through the gut because of the strong magnetic force of attraction

Parents need to watch out for

1. Chest pain, foreign body sensation in the food pipe (the feeling of something being stuck )
2. Gagging, vomiting and refusal to feed
3. Difficulty in breathing and cough
4. Vomiting blood
5. Stomach pain and blood in stools
6. Rash and itching

Parents have to consult the doctor atleast once. A Xray may be ordered. High risk foreign bodies like button batteries lodged in esophagus may need to be urgently removed using an endoscope. Other high risk objects too may need intervention. Most of the times, parents will be asked to observe for the object in the stools, to a maximum period of three weeks ( provided there are no other issues like pain and vomiting). There is no role for laxatives.

Please Note : ONE SHOULD NOT INDUCE VOMITING AT ANY COST. IT CAN LEAD TO DANGEROUS COMPLICATIONS.

PREVENTION IS BETTER THAN CURE!

PLEASE DO NOT KEEP HIGH RISK OBJECTS LIKE BATTERIES, MAGNETS AND SHARPS AROUND CHILDREN.

At lower levels of exposure Lead causes no obvious symptoms. At high levels of exposure lead attacks the brain and central nervous system, causing coma, convulsions and even death. Dear parents *Diarrhoea* is the most common problem in Summer. *Dehydration* is an important complication of Diarrhoea. Give buttermilk, fresh fruit juice, lemonade, coconut water and ORS to prevent dehydration. *ORS* should be based on *WHO formula*. Many brands of ORS ( tetra packs ) are not having this composition, which when used may cause *electrolyte imbalance and worsens dehydration*. So whenever you buy ORS look for _based on WHO formula _ written over it.

*Child sitting in W position*
Is it something to really panic about?

Not really, provided your child has no hip related issues and/or increased muscle tightness .

W position, cross legged position and other sitting positions; all are ok provided the child is not sitting in any one position all the time/for prolonged periods.

Sitting only in W position all the time can affect core muscle strength , hinder cross body movements and delay the development of hand preference (hand preference is very essential for future writing ability).

There is no need to pressurise the child not to sit in W position, but one should definitely encourage the child to sit in other positions too.
