Successful Management of Esophageal Perforation with Fully Covered Self-Expanding Metal Stent.
A 75-year-old male with a history of intermittent dysphagia due to peptic stricture underwent dilatation for a mid to distal esophageal stricture. Two years prior, he had undergone similar dilatation. After the recent procedure, the patient experienced sudden onset dysphagia and was diagnosed with esophageal perforation.
Upon arrival at our quaternary care center, the patient was stabilized and underwent endoscopy, revealing a large perforation (34-37 cm from incisors). A fully covered self-expanding metal stent was placed, sealing off the perforation.
We had to do a repeat procedure and fix the stent after 2 weeks as the stent migrated distally.
Good news is we could see the size of the perforation becoming very small when we fixed the stent.
*Outcome*
The patient's condition improved significantly after stent placement. This case highlights the importance of prompt intervention and the effectiveness of fully covered self-expanding metal stents in managing esophageal perforation and whenever necessary dont hesitate to fix your stent during the first procedure itself.
