EVERY TRANSPLANT HAS A STORY , some simple , and SOME LIKE A BLOCKBUSTER - THIS ONE MY 33rd BEAT ALL - A 14 yrs child with DCMP , severe MR , Hugely dilated heart , EF of 18 %, listed a month back , but used to attend OPD nearly every other day with SOB , , accompanied by a father who was helpless , attended by me who was equally helpless. Infact I started avoiding meeting , and kept myself emotionally dissociated. 2 days back my team members said he was very breathless, in emergency, admitted in the children’s icu early morning . I reached the hospital at 10 am for our regular case an MICS CABG . was checking my WhatsApp messages , saw a message in our transplant group from our coordinator. This was at 9.30 am . Surprised why nobody called me, called the coordinator , without going through the reports . He said our Anaesthesist said we won’t take the heart as it was a marginal donor . Our recepient was 14 yrs , 36 kgs ., 1.5 mts . EF AROUND 10 %. The donor was 37 yrs , poly trauma , 75 kgs , 1.65 mts. EF OF 40 %. . There was a 55% mismatch . I told him we will take the heart .then I called my anaesthetist , who is more adrenergic and dynamic than me 😀, if there can be one , why he said no . I reasoned that 40 is any day more than 10 . Either way the child won’t come out . This was a small chance . my Anaesthetist, reluctantly agreed .then we spoke with the Father , explained the marginal donor heart problem, the minimal chance of survival , but a CHANCE . The poor guy didn’t have a choice , tearfully said ALLAH WILL TAKE CARE . We started getting ready . I completed my Mics case . Then suddenly we got a call from the PICU , the child was having convulsions. We rushed, the child was having GTCS. Given all drug’s , but no response , with a 10% Ef, our Anaesthetist intubated . Now we were left with a kid whose neurological status was also not known. In the meantime came to know that our hospital had exhausted the stock of SIMULET , aDRUG ,used for transplant induction . Now we had a patient on a ventilator with , unknown neurological status , ready for transplant induction, but no induction drug . Asked all around , no hospital had stock. Contacted one pharmacist , who said he will give it on the sidelines. Sent my medical officer on a Scooty to collect it with money . The gentleman refused money and gave the injection. Advised my resident to drive back safely ( was more worried for the injection, than him😀). Didn’t want any more twists . I PRAYED TO GOD THAT I WILL COMMIT A FEW LESSER SINS FOR THE COMING FEW DAYS . The HARVEST AND SURGERY went smoothly , the HEART REGAINED SINUS RYTHM IMMEDIATELY . Returned at 1 am yesterday, had a stiff peg and waited . around 3am my colleague called and said CHILD AWAKE, NEUROLOGICALLY FINE . and then I cried and SLEPT OFF . WE HAD DONE IT . STILL A LONG WAY TO GO , BUT HE HAS SEEN 2 SUN RISES , and hopefully many more
