Year 2025 has been ”Road to Damascus” moment in my life and professional journey.

I have changed workplace in Jan 2025 with responsibility to start neurosurgical center from scratch. I have been in neurosurgery since 2011 and understood it well “performance is persuasion and results are final authority.” Reputation moves faster than you. Competence anchors you. Resources protect you. Legitimacy shields you. Relational influence multiplies you.

“Sinking Skin Flap Syndrome- Post Craniectomy”- rare sequele of decompressive craniectomy where skin flap sinks inward due to atmospheric pressure hindering neurological recover or sometimes even worsening.

In this patient, Emergency brain hemorrhage surgery was performed as life saving measure. At 3 months, he improved but can not return to previous life roles. It’s was spasticity which was hindering rehabilitation process. With discussion, we performed cranioplasty for cosmetic purpose but improvement in muscle function was cherry on the Top. At the time of suture removal he came alone in chamber without any support. Gratitude in his eyes further strengthened my belief in miracles of neurosurgery.

“Microsurgical clipping of ruptured Acom aneurysm”

In continuation of my previous post as dual trained cerebrovascular surgeon, I will highlight comprehensive care for ruptured brain aneurysms. Patient is young female( 28/f), who have drug resistant hypertension (on 3 anti hypertensives) and chronic kidney disease. She also suffered hemorrhagic stroke in left basal ganglia territory 5 years back. Her old angiogram didn’t show any aneurysm.

Now she came to emergency in altered sensorium. Ct scan showed SAH and IVH. Her Ct angiogram showed berry aneurysm at Acom artery. Her base line creatinine was 1.8-2.5.

Considering her age, CKD, Bovine arch ( left CCA branching off brachiocephalic trunk) with tortuous cervical ICA and wide neck aneurysm we offered microsurgical clipping clearly advantageous in this scenario. After clipping patient remained neurological stable. Regular follow up maintained with nephrology and cardiology. Her creatinine remained in same range. SAH sequele were predictable and managed as per standard guidelines.

Now she is back to normal from neurological point.

You guys made it possible to perform two different microsurgery and endovascular case on same day. *It’s true form of Holistic care for cerebrovascular diseases*. Despite patients arriving to us in critical state, we are able to deliver optimum- *THE BEST POSSIBLE.*

Endovascular case- Ruptured fusiform dysplastic right vertebral artery aneurysm managed with *Flow diverter stenting Pipeline Shield Flex*. Perfect positioning is important to spare opposite vertebral artery and critical perforators at vertebra-basilar junction medullary perforators.
Patient had uneventful hospital stay.

Microsurgical clipping case is very complex scenario but we delivered good result for patient. It will be shared in next case with surgical video.

Calcified lumbar PIVD surgery
Life returned to normal

Patient who is professionally related to construction work has been suffering from severe back pain and lumbar radiculopathy. Slowly he have to leave his job. He was reffered to us with MRI on which it looked like routine lumbar PIVD case but ct scan revealed dystrophic calcification of herniated disc fragment/overlying PLL . Microscopic approach provides 3D Visualisation of anatomy and higher degree of freedom for meticulous pin point dissection. Patient became completely symptoms free since the day of surgery and returned to his profession on follow up.

Living independently without precautions and medicines.

Dinasaur 🦕 shaped Ruptured MCA aneurysm clipping 🎈 🔖

🚨 27/f with Left insular SAH and ICH, finances was not an issue but family members wanted long term results due to young age.

👨‍⚕️ After Multidisciplinary discussions with IR-INR-Neurology-Neurosurgery, Microsurgical aneurysm clipping done.

Surgery and Postop hospital stay remained uneventful.

Also Note No RADIOLOGICAL SCAR (GLIOSIS) in f/u CT scan.
