Black and white images of CT/MRI are not enough to measure the success of procedure. The way Neurosurgical procedures alters the natural history of the many lethal diseases is most amazing part of this speciality.

This Smile, Confidence on the face of patient during follow up is more satisfactory to me in addition to ct/mri success. Patient herself asked me to click a picture for helping out in difficult phase of life.

Craniotomy part is basic neurosurgical skill, but if it’s done with plastic surgery level cosmetics along with meticulous intradural work then there is nothing more beautiful speciality and it becomes NEUROSURGERY HEALS.

NEUROSURGERY HEALS

Operation like this reminds, why I become Neurosurgeon.

*Giant Olfactory Groove Meningioma (6.5cm) presenting as Foster Kennedy syndrome*- got operated and cured ( removed the tumor + involved DURAMATER attachment + bony overgrowth). Patient
is cured of her illness.

In this 10 hour long surgery, we are thankful to team. Not only me but Even the OT staff didn’t take any break during whole duration of surgery.

“Best Smile ever after getting triumph over the Chronic illness”

This is my tribute to hardworking suburban mothers and my mother too because I have seen this from childhood to collage days. It’s common process that every middle age female has to go through in Indian countryside life. Role of mother is to look after kids, husband and old parents along with farming. It’s hard labour without any payment. An unbreakable cycle without any off day (including festivals) which can only be broken by next generation. If somebody is able to raise kids and educate them then only this cycle of endless labour ends.

This story is narrated by patients SON who have recently joined govt job and shifted to state capital. She tolerated pain for a decade but with time pain became unbearable and causing sleepless nights. She got investigated and planned for surgery. Even the day before surgery she was worried about the cow and rain. She will start crying in every little conversation we had before operation. My focus is not only smooth hospital course or few weeks follow up but the complete return to life.

Two level TLIF and complete decompression of four exiting nerve roots on both sides was done.

From painful walking in bending position she is able to stand upright without pain. Now she herself gained the confidence to standup and smile. Her Son like many of us from countryside roots have broke the cycle of apathy and ignorance. 👍

This story is about what single person can go through in one life and still come out strong.

32 years old Male who survived severe head injury 10 years back, developed new hemiplegia due to hemorrhagic stroke. Cause for hemorrhagic stroke was CKD associated hypertension. At his local district he was intubated for low oxygen saturation due to aspiration. Low urine output restricted use of cerebral decongestants.

Family have been counselled for prognosis due to accumulated problems at district hospital.

From Neurosurgical perspective, our aim was to provide option which require minimum antibiotics ( restricted due to CKD), resolve raised intracranial pressure without need of cerebral decongestants, minimum cortical footprint with good control of bleeding ( to prevent re-expansion due to heparin used in dialysis) . Nevertheless, As team we managed to cope with all the tasks.

Neurologist ( Dr Ravi Jakhar), Anaesthesia Team, INTENSIVISTS, Cardiologist ( Dr Anil Choudhary), Nephrologist, rehabilitation specialists ( Dr Sweta) helped the patient to quickly getting stable hemodynamics, stable kidney function, weaning off and finally getting out of bed.

We tried to restore Brain function to lead independent life. Optimum drug therapy is choosen for kidney disease and hypertension.

यह कहानी इस बारे में है कि एक व्यक्ति अपने जीवन में किन-किन परिस्थितियों से गुज़रकर भी मज़बूती से उभर सकता है।

32 वर्षीय पुरुष, जो 10 साल पहले सिर में गंभीर चोट लगने के बाद भी जीवित रहा, रक्तस्रावी स्ट्रोक के कारण नए हेमिप्लेजिया से पीड़ित हो गया। रक्तस्रावी स्ट्रोक का कारण किडनी की बीमारी से जुड़ा उच्च रक्तचाप था। अपने स्थानीय ज़िले में, एस्पिरेशन के कारण कम ऑक्सीजन संतृप्ति के लिए उसे कृत्रिम सांस के लिए वेंटीलेटर पर लिया गया था। कम मूत्र उत्पादन के कारण दिमाग़ की सूजन कम करने वाली दवाओं के उपयोग पर प्रतिबंध होता है |

न्यूरोसर्जिकल दृष्टिकोण से, हमारा उद्देश्य ऐसा विकल्प प्रदान करना था जिसमें न्यूनतम एंटीबायोटिक दवाओं (किडनी की बीमारी के कारण सीमित) की आवश्यकता हो, दिमाग के सूजन कम करने वाली दवाओं की आवश्यकता के बिना बढ़े हुए इंट्राक्रैनील दबाव को कम किया जा सके, रक्तस्राव पर अच्छे नियंत्रण के साथ न्यूनतम कॉर्टिकल फ़ुटप्रिंट (डायलिसिस में प्रयुक्त हेपरिन के कारण ब्लड क्लॉट के पुनः विस्तार को रोकने के लिए)। फिर भी, एक टीम के रूप में हम सभी कार्यों को सफलतापूर्वक पूरा करने में सफल रहे।

न्यूरोलॉजिस्ट (डॉ. रवि जाखड़), एनेस्थीसिया टीम, इंटेंसिविस्ट, कार्डियोलॉजिस्ट (डॉ. अनिल चौधरी), नेफ्रोलॉजिस्ट, पुनर्वास विशेषज्ञ (डॉ. श्वेता) ने मरीज़ को जल्दी ही स्थिर ब्लड प्रेशर , स्थिर किडनी फ़ंक्शन, वेंटीलेटर के बिना साँस और अंततः बिस्तर से उठने में मदद की।

हमने स्वतंत्र जीवन जीने के लिए मस्तिष्क की कार्यक्षमता को बहाल करने का प्रयास किया। गुर्दे की बीमारी और उच्च रक्तचाप के लिए सर्वोत्तम औषधि चिकित्सा का चयन किया गया है।

Desperate aggresive action brought positive results. If I would have been performing the first surgery of same patient then won’t plan it but it’s redo, nature of disease and desperation of family which prompted to take this aggresive approach.

Patient is following up with oncology doctors. Recently got hospitalised for epistaxis and locally mri brain was done. They just whatsapped mri images.

He is alive, slow but moves all limbs, stays indoor
But doesn’t have seizure on single antiepileptics.

Any thoughts?
