Good morning respected Seniors and dear colleagues,

Sharing a interesting and different case .

It looks very difficult but treated with simple management

7 year old girl

Single episode of seizure
Imaging showed partially thrombosed giant dysplastic intradural (V4) segment of left vertebral artery causing ventral compression over medulla.

On 3D DSA- left PICA and anterior spinal artery was arising from aneurysm dome and anterior aspect respectively.

Safest treatment was proximal occlusion of parent artery.

Endovascular coiling of proximal aspect of V4 segement of left vertebral artery (PVO) was done just proximal to aneurysm. Aneurysm is near completely obliterated.

Check angio showed retrograde filling of left PICA from right vertebral artery and reconstitution of anterior spinal artery from proximal left vertebral artery.

Patient is doing well.
No new deficit.
