A 72‑year‑old male presented to us with an acute AWMI. CAG revealed significant stenoses: 90% at the ostioproximal LAD artery, 60-70% in the mid‑LAD, and 90% ostial stenosis of the first diagonal branch.
A bifurcation PTCA of the LAD–D1 was planned using a two‑stent strategy. Drug‑eluting stents (2.75 × 16 mm to the diagonal branch, and 3.0 × 36 mm to the LAD) were successfully deployed. Final kissing balloon inflation was performed with balloons sized 2.5 × 12 mm in diagonal and 3.0 × 12 mm in LAD. Post‑procedure, TIMI grade III flow was achieved, with an optimal angiographic result.
