Mechanical THROMBECTOMY and thrombolysis is the treatment of choice for acute stroke, it is done in cathlab without open surgery or in emergency it self.

Good morning friends

Sharing a interesting and different case .

It looks very difficult but treated with simple management

7 year old girl

Single episode of seizure
Imaging showed partially thrombosed giant dysplastic intradural (V4) segment of left vertebral artery causing ventral compression over medulla.

On 3D DSA- left PICA and anterior spinal artery was arising from aneurysm dome and anterior aspect respectively.

Safest treatment was proximal occlusion of parent artery.

Endovascular coiling of proximal aspect of V4 segement of left vertebral artery (PVO) was done just proximal to aneurysm. Aneurysm is near completely obliterated.

Check angio showed retrograde filling of left PICA from right vertebral artery and reconstitution of anterior spinal artery from proximal left vertebral artery.

Patient is doing well.
No new deficit.

Brain haemorrhage
(Subarachnoid haemorrhage ) Aneurysm coiling case-

Sharing a wide neck multilobulated ruptured ACOM aneurysm, three vessels at neck of aneurysm (separate division of pericallosal and callosomarginal arteries incorporating aneurysmal neck and heubner artery)

54 yr old

WFNS-1
Gyrus re**us bleed and IVH- grade 4SAH

Challenge was to preserve all the arteries from this wide neck aneurysm

Cauliflower like aneurysm anatomy

Balloon assisted coiling done successfully.

Patient is doing well post procedure.

Sharing a interesting and challenging case

60 yr gentleman,

Diffuse SAH and mild IVH

Ruptured right V4 dissecting aneurysm (looks Pseudo fenestration)

Treated with Endovascular Flow divertor Stent to save right PICA.

Patient extubated post procedure without deficit.

Recurrent stroke (speech difficulty) in left MCA territory.

DSA shows critical stenosis (>95%) or blockage in left internal carotid artery origin (major vessel of brain)

Angioplasty and stenting is done using Filter (EPD) device for distal protection.

Post procedure good blood flow through the stent and distal ICA and it's branches.

Patient is fine after post procedure.

Grade 3 SAH

WFNS -1

Brain haemorrhage due to ruptured SCA aneurysm
Relatively wide neck and challenge to preserve Left superior cerebellar artery which is also including aneurysm neck.

Did endovascular coiling (without opening the skull)
Without any stent/balloon.

Post procedure, aneurysm has secured by coils with patent SCA.
Patient is fine without any deficit.
