Case Summary:
15-year-old boy with severe rigid bilateral pes planovalgus, left side worse, presenting with significant difficulty in walking.

Surgical Management:
Corrected using a minimal-incision double fusion:
• Sinus tarsi subtalar fusion
• Medial talo-navicular fusion with plate fixation
• Heel valgus corrected and maintained using a tricortical bone graft

Outcome:
At 4 months, the patient is walking comfortably with a plantigrade foot and no residual deformity.
