Plz try to understand RTH Bill
कृपया समझने की कोशिश तो कीजिये

RTH bill से पहले

मुझे रात में २ बजे सीने में दर्द हुआ । आनन फ़ानन में अपने शहर के सबसे अच्छे कार्डियोलॉजिस्ट के हॉस्पिटल पहुँच गया। वहाँ पहुँचते ही नर्सिंग स्टाफ ने मुझे देखा और जिस कार्डिलॉजिस्ट को में दिखाना चाहता था उस डॉक्टर को फ़ोन कर दिया। डॉक्टर अपने घर से कुछ ही समय में हॉस्पिटल पहुँच गया। उसके आने से पहले ही मेरे सारे टेस्ट हो चुके थे। high tech मशीन से कुछ ही मिनट में मेरे सारे रिज़ल्ट्स भी आ गये । चूँकि हॉस्पिटल महँगा था इसलीय वहाँ ज़्यादा भीड़ नहीं थी और वो हॉस्पिटल दिल के मरीज़ों के लिए ही था इसलीय भी वहाँ ज़्यादा patient नहीं थे । मुझे तत्काल ही ICU में shift कर दिया गया। डॉक्टर ने सारा ट्रीटमेंट किया। उनके पास इतनी हाई टेक मशीन थी जिससे बिलकुल कम दर्द के साथ सटीक इलाज किया जा सकता था। आईसीयू में भी डॉक्टर और नर्सिंग स्टाफ बस एक बटन दबाने पर ही आ जाते थे। साफ़ सफ़ाई इतनी की फ़र्श पे अपना चेहरा देख सके। मशीन इतनी अच्छी की सारे रिज़ल्ट्स कुछ ही मिनट में आ जाते थे। हाँ पैसे लगे उस हॉस्पिटल में पर वो पैसे मेरे हेल्थ इंश्योरेंस से कट गये। पर इलाज में कोई कमी नहीं थी। और कुछ ही दिन में मैं ठीक होकर घर आ गया।

RTH बिल आने के बाद

रात को २ बजे मेरे सीने माँ दर्द हुआ। आनन फ़ानन में मैं अपने शहर के सबसे अच्छे कार्डियोलॉजिस्ट के हॉस्पिटल पहुँचे गया। वहाँ जाकर देखा तो पता चला कि मेरी ही जैसे इमरजेंसी में मुझसे पहले ही बहुत सारे पेशेंट आये हुए हैं। चूँकि वो हॉस्पिटल बस दिल के मरीज़ के लियें था पर इमरजेंसी में उनके पास दांत के दर्द, पेट के दर्द, सिर दर्द जैसे मरीज़ भी इमरजेंसी के रूप में थे। काफ़ी देर इंतज़ार करने के बाद मेरा नंबर आया तो पता चला कि वहाँ दूसरा डॉक्टर इलाज कर रहा है। मैंने पूछा वो फेमस कार्डियोलॉजिस्ट के बारे में तो पता चला कि वो अपनी १२ घंटे की ड्यूटी पूरी करके अभी गये हैं । अब हॉस्पिटल के पास सारे फ्री पेशेंट आते हैं तो हॉस्पिटल उनको ओवर टाइम करने के लियें पैसे नहीं दे सकता। मैं दूसरे हॉस्पिटल जाने की सोचता पर अब शहर में कोई और अच्छा अस्पताल ही नहीं था। बहुत सारे अस्पताल RTH आने के बाद बंद हो गये थे। मजबूरी में मुझे दूसरे कार्डियोलॉजिस्ट को ही दिखाना पड़ा। उन्होंने मुझे कुछ टेस्ट लिख के दे दिये । जब वो टेस्ट करवाने गया तो वहाँ भी फ्री इमरजेंसी पेशेंट की भीड़ थी । नर्सिंग स्टाफ भी कम था या शायद पेशेंट ही इतने ज़्यादा थे। मेरी बारी आने पर मैंने टेस्ट करवाए और मुझे बोला गया की रिपोर्ट १ घंटे बाद आएगी। क्योंकि हॉस्पिटल के पास अब हाई टेक मशीन नहीं हैं। और मुझसे भी पहले और पेशेंट की report भी देनी है। रिपोर्ट आने के बाद इलाज के लिए भी हॉस्पिटल में कोई भी हाई टेक मशीन नहीं थी। बहुत ही पुराने तरीक़े से मेरा इलाज किया गया। जिसमे समय भी बहुत लगा और बहुत सटीक भी नहीं था। इसके बाद आईसीयू में भी फ्री इमरजेंसी वाले patient की भीड़ थी। कहीं भी जगह नहीं थी। शायद मेरी क़िस्मत अच्छी थी इसलिए जैसे तैसे करके एक बेड मिला। ICU में इतने ज़्यादा पेशेंट थे की डॉक्टर और नर्सिंग स्टाफ बहुत ही देर में आते थे । बार बार सफ़ाई करने के बाद भी सफ़ाई दिखती नहीं थी क्योंकि लोगो की भीड़ का ताँता लगा हुआ था। पर इलाज के लियें पैसे नहीं देने पड़े। पर इलाज में देरी के कारण जो नुक़सान शरीर को होना था वो हो गया।
ये हमारे शहर का सबसे अच्छा pvt हॉस्पिटल था। RTH aane के बाद इसकी हालत govt हॉस्पिटल जैसी हो गई थी। हर कोई इमरजेंसी के नाम पे फ्री इलाज के लिए ही आया था । हॉस्पिटल किसी को भी मना नहीं कर सकता था। हॉस्पिटल अब इतने पैसे नहीं कमाता जिससे वो और डॉक्टर, नर्सिंग स्टाफ को नौकरी दे सके। कम सैलरी मिलने के कारण सारे अच्छे डॉक्टर ने दूसरे राज्य में अपने हॉस्पिटल खोल लिए।
हाँ सरकारी हॉस्पिटल अभी भी हैं। वहाँ सरकार को तरफ़ से बहुत पैसा भी आता है । अच्छी मशीन भी हैं पर रख रखाव के अभाव में सारी ख़राब पड़ी हैं। पहले त्वरित और अच्छे इलाज के लिए हम pvt अस्पताल जाते थे । पर अब या तो pvt अस्पताल ही इतने कम थे और जो थे वहाँ भी सरकारी अस्पताल जैसी भीड़ लगी रहती है। स्पेशलिटी जैसा कोई अस्पताल अब है ही नहीं क्योंकि कार्डियोलॉजिस्ट भी दांत का मरीज़ देखता है जो उसके पास इमरजेंसी में आया है।

The most commonly asked question by my pregnant mothers _

How do I plan for my delivery/admission to hospital with rising Covid cases?

1.Strict social distancing within 2 weeks from expected due date ,37 weeks in maximum patients.If feasible opt for work from home,avoid unnecessary travel.

2.All preoperative investigations, Covid testing preferably to be done a day before planned admission.

3.Only 1 attendant to accompany each patient,avoid attendant switching.Children and elderly strictly not allowed.

4.Wear surgical mask throughout your hospital stay.

5.Call early if complaints of reduced baby movements,pain abdomen,discharge or bleeding so that Covid testing is scheduled without delay with other treatment in time.

6.With rising numbers in young and children you need to be more vigilant.Other than the complications in the mother and the baby,difficulties during isolation and the discomfort of possibly not being able to deliver in the hospital of your choice are also reasons to worry.

How do I know about my risk of having Breast cancer?

Risk depends upon age,ethnic group(Asian),weight, height, age of 1st period,Age of stopping periods(if already menopause),age when 1st child was born,1 or more children.

Please see your doctor if more than 5 are yes.
1.Alcohol intake daily
2.Smoking
3.Breast cancer in family (mother ,daughter ,sister ,aunt_mother or father's sister) esp. if less than 50 yrs.age
5.You had uterine cancer
6.You had ovarian cancer
7.Do you have gene mutation/changes linked to breast cancer.
8.Dense breasts on Mammogram/taking COCs pills
9.You had radiation therapy to chest
10.History of intake of DES drug by your mother when pregnant.

If more than 45 yrs and any risk factor,should consult your doctor

Some risk factors like age,family history and no full term pregnancy cannot be changed.
But you can change others like weight, smoking and alcohol.
Avoid hormones after menopause or restrict to lower doses /short periods.
Follow healthy diet and regular exercise.

Having risk factor does not mean that you will develop Breast cancer.But having risk factors is a good reason to discuss with your doctor and schedule regular screening mammograms.

Screening advice for women at average risk
1.All women should know the benefits,/limits/potential harms of Breast Cancer Screening.
2.Should know about their breasts normal look and feel and report any changes.
3. 40_44 yrs=choice to start mammogram annually
45_54 yrs =mammogram yearly
More than 55 yes.=mammogram once every 2 years/continue yearly screening.

Screening should continue as long as women are in good health or expected to live at least 10 more years.

Vaccination in pregnancy _ TT and beyond

Vaccines help in protection of mother's body from infection and immunity passes to her baby during pregnancy.The immunity keeps the child safe during 1st few months of life until baby gets his own vaccination.
Vaccination is not aimed at increasing the intelligence or the weight of the baby, or make the baby more fair.

Other than Tetanus Toxoid(TT), Influenza,Hepatitis B,Tdap are advised in pregnancy.

Avoid Chickenpox vaccine ,MMR vaccine,HPV vaccine during pregnancy.
Please consult your obstetrician if you conceive within 3 months of taking these vaccines

Vaccines will be further discussed briefly in the coming weeks.
Do not miss your vaccinations due to Corona.Take all precautions and visit your obstetrician as advised..
