SHOULDER IMPINGEMENT

➡️Shoulder Impingement is a very common injury.
➡️In this post I will focus on the Supraspinatus, one of our rotator cuff muscles that is mostly the culprit.

➡️The supraspinatus tendon is running through a space in between the acromion and the humeral head which we
call the subacromial space.

➡️Note how tight this space is, this is where Impingement occurs, which can also affect the subacromial bursa and the long head of the biceps brachii.

➡️When you move your arm up in abduction, the subacromial space
becomes narrow and causes entrapment of the supraspinatus tendon causing irritation of the tendon or bursa.

➡️Doing repetitive overhead movement can aggravate your tendon making it inflamed & thickened.

➡️When it’s thickened, the subacromial space will become even more narrow! As a result you get reduced range of motion,
pain and potentially a supraspinatus tendon tear.

‼️Who’s at risk?
➡️Athletes who do lots of arm overhead activity (swimming, javelin, tennis, basketball) or any activity that involves
repetitive movement (construction work, wall painting).

📌Symptoms:
🔸️Pain when moving overhead, lifting heavy objects, or reaching for an object.

🔸️Possible swelling and tenderness in front of the shoulder.

🔸️Reduced range of motion due to pain and stiffness.

🔸️Weakness of rotator cuff muscles and possibly other surrounding muscles. Sometimes you’ll be able to see loss of muscle bulk too on the affected side.(chronic stage).

🔸️Pain at night (think about sleeping position).

➡️Normally, at the early stage (when impingement is still mild), people will tend to ignore it until they have caused further injury/tear, that’s the time when
they will seek for treatment.

‼️Never force yourself through pain, the pain is there for a reason!

✅️Consult a therapist if you’re in doubt! 📌They can confirm with you whether you have an impingement by doing
some special tests and guide you on a proper pain management and rehab program.

The Scapula
➡️Due to the large surface area of the scapula there are a large number of muscles attached (17 in total) which fixes the scapula to the thoracic wall and allow it to move.
➡️These muscles are summarised below and are separated based on muscles
originating or inserting onto the scapula.

➡️Four of these muscles form the rotator cuff, which covers the shoulder
capsule (subscapularis, infraspinatus, teres minor and supraspinatus).

📌Muscles that ORIGINATE from the scapula:
🔸️Deltoid
🔸️Supraspinatus
🔸️Infraspinatus
🔸️Teres minor
🔸️Teres major
🔸️Subscapularts
🔸️Triceps brachii (long head)
🔸️Biceps brachii (long & short heads
🔸️Coracobrachialis
🔸️Omohyoid
🔸️A small part of the latissimus dorsi

📌Muscles that INSERT on the scapula:
🔹️Trapezius
🔹️Levator scapulae
🔹️Rhomboid major
🔹️Rhomboid minor
🔹️Serratus anterior
🔹️Pectoralis minor

📌Fractures:
➡️Like any bone, the scapula is subject to fractures.
‼️However, because the scapula is well protected they are uncommon, representing 0.5 to 1% of all fractures.

➡️Anteriorly, the scapula is protected by the rib cage and thoracic cavity while posteriorly covered with a lot of soft tīssue (i.e. muscle).
➡️Therefore scapular fractures usually occur as a result of high-impact direct
trauma and nearly all of the incidences are associated with other much severe and sometimes multiple and life-threatening injuries.
➡️Because of this, scapular fractures tend to go undiagnosed until later and therefore the treatment for scapula fractures is delayed.

Relieving Frozen Shoulder with Physiotherapy

What is Frozen Shoulder?
Adhesive capsulitis (frozen shoulder) leads to pain and stiffness, restricting shoulder movement and affecting daily life.

Key Physiotherapy Approaches:
• Joint Mobilizations: Gentle techniques to reduce stiffness.
• Stretching: Pendulum exercises, cross-body stretch, and sleeper stretch to restore mobility.
• Strengthening: Resistance band training to rebuild shoulder strength.
• Modalities: Heat or TENS therapy to ease pain and prepare for exercises.

Consistency is Key: Regular exercises and movement are crucial to recovery.

MUSCLE CRAMPS 💥

1. A muscle cramp happens when a muscle contracts involuntarily and forcefully, and it does not relax.

2. Muscle cramps can affect any muscle, though they are most common in the legs and feet.

3. Nearly everyone experiences muscle cramps at some point in their lives.

4. Muscle cramps have various causes and can occur in different forms.

5. They may happen during physical activity, while resting, or at night, depending on the underlying cause.

6. Dehydration is one of the most frequent triggers of muscle cramps.

7. Certain medications can also lead to muscle cramps.

8. Stretching the affected muscle often provides relief and stops the cramp.

9. Proper hydration, nutrition, and careful exercise practices can help prevent muscle cramps, along with paying attention to ergonomic factors.

WAYS TO TREAT AND PREVENT LEG CRAMPS 🦵

a) Take a warm, relaxing bath before bedtime to loosen tight muscles.

b) Apply a heating pad to the area where the cramp occurs.

c) Consider dry needling therapy to relieve tightness in leg muscles.

d) Ensure your body has sufficient levels of magnesium and potassium.

e) Stretch your leg muscles before going to sleep.

f) Avoid wearing high heels and opt for ergonomic footwear instead.

g) Slowly rise and walk around when you experience cramps to improve blood circulation.

h) Massage or use a foam roller for the cramped muscle with gentle, circular movements.

i) Training your muscles regularly to decrease or prevent muscle cramps.
