Case Of:- OA Knee Severe Varus Deformity with medial side Bone Defect.
Done with Extension Rod in tibia as the Shaft tibia had a Stress Fracture.

Case Of OA knees :- TKR done!

Case of a:- Fracture Neck Of Femur in a old age Patient.

Hemi Arthroplasty was done!
Pre/ Post Op

Post op Patient was made Ambulatory the Next post op day.

Case of Fracture:- Segmental Fracture Tibia.
3 Year Follow up!

Intramedullary Nailing was Done with Proximal Plating For Absolutely Stability and Compression in the Proximal Fragment and also to allow weight bearing the next Post op day!

The Main Problem encountered in this case was the Union of the Fibula before the union of the Shaft Tibia so After 7 months post op, Proximal Fibula Fracture was made and patient was made Ambulatory The Next post op day
and no bone Grafting was done and Now.

would you have done the same?
or just Use the Pollar Screws and what would have been your weight Bearing protocol ?

Case of Fracture:- A Dislocated Ankle with Bimalleolar Fracture with Severe Soft tissue injury in a elderly Patient

Ankle was reduced and soft tissue was let to heal.
Second stage Minimally invasive ankle arthrodesis with a retrograde Nailing was done!

Pre and post op xrays
Patient was made ambulatory the 2 Post of day!

PFN! Done in LATERAL DECUBITUS POSITION.
Pre and Post op X-ray
A Cancellous screw was put To anchor the broken GT.

Case of Fracture:- Proximal Tibia Undisplaced type!
But it's a Stress Fracture which got converted to a Full Fracture with a Simple Fall. Thanks to Simon Donald for Pointing it out!

IM Nailing done! Pre and Post op X-ray
Patient was Made Ambulatory and Full weight Bearing the Next post op day!

To my Surprise as this OT was Already done! but Brent Dressler, DO, MHA, MLS was perfect in predicting the Canal size and reamer to be used and the Nail size.

The canal was reamed till 10mm and a 9mm nail was put as & when the Lateral cortex made a bone to bone contact so much of Back Slapping wasn't done.

Case of Fracture :- Mid Shaft Tibia/Fibula
1Year and 4 months post op Follow up!
Patient is pain Free and Full range of motion!

Dynamization was done after 3 weeks of surgery.
Patient was Full weight Bearing from Day 2 Post op!

Tibia Shaft fractures are very common!
Mode of treatment sometimes depends upon the Type of the Injury and also subjected to the Skin Condition.
But still IM Nialing is the most common Mode of Treatment.

Case of Fracture:- Proximal Shaft Tibia
1 Year Follow up!

MIPO Plating was done!

Patient was made ambulatory the Next post op day with Full range of Knee Exercises.
Now After 1 year!
Pain Free and able to Sit, Squat and Sit crossed leg.

Case of Fracture:- 1 YEAR Follow up!

This Fracture is somewhere between the Pertrochanteric region.
AO classification 31 A1
a Stable Interthrochanteric type Fracture! In a elderly lady.

Pre and Post op X ray with 1 year Follow up x rays!

Patient was made Ambulatory the next post op day.
No Displacement or the loss of reduction or loosening of Implant in a elderly Osteoporotic Patient has been noted in 1 year.
So even now DHS holds some place in Orthosurgery, But one has to be very selective and the amount to Lateral wall should also the thick to have a Good outcome.
