Shree Krishna Hospital Achieves Landmark Success in Complex Liposarcoma Surgery.

Shree Krishna Hospital, Karamsad, recently achieved a remarkable surgical feat with the successful removal of an 8.5 kg retroperitoneal liposarcoma from a 55-year-old male patient. This case was particularly significant given the patient's severe pre-existing medical conditions and the tumor's massive size, which had previously led to assessments of inoperability.

The patient, suffering from a significant history of ischemic heart disease and severe left ventricular dysfunction (ejection fraction of 30%), had experienced a progressive abdominal mass for three months. Multiple external medical opinions had deemed the case inoperable due to the tumor's sheer volume and the patient's compromised cardiac function.

Upon referral from a private hospital in Nadiad, the patient was admitted to Shree Krishna Hospital, a tertiary care center. Dr. Raghunandan GC, Surgical Oncologist, stated that initial evaluation revealed an extensive retroperitoneal liposarcoma stretching from the xiphisternum to the pelvis and inguinal canal, causing complete displacement of abdominal structures.

Under Dr. Raghunandan GC's expert leadership, the challenging surgery was meticulously performed. He was crucially supported by the General Surgery Department team, (Dr. Akshath Vadaaliya and Dr. Kamlesh Kamaliya). The patient underwent comprehensive optimization prior to the complex procedure. Despite the inherent difficulties, the anesthesia team—(Dr. Vaibhavi Hazariwala, Dr. Nirali Panchal, Dr. Jishmi Shah, and their teams)—skillfully managed the case, ensuring the patient's safety throughout the surgery and a smooth emergence without complications. Effective postoperative pain control further contributed to a comfortable recovery.

This successful outcome underscores Shree Krishna Hospital's unwavering commitment to tackling highly complex medical challenges through an interdisciplinary team approach, encompassing surgical, anesthesia, and critical care teams, and a holistic, patient-centered strategy. It highlights the critical importance of appropriate referrals to tertiary care facilities, where specialized expertise and collaborative efforts can lead to positive outcomes even in the most daunting cases.

Shree Krishna Hospital Saves Esophageal Cancer Patient After Complex Interventions.

A 42-year-old man, battling advanced esophageal cancer, has made a remarkable recovery and has been discharged from Shree Krishna Hospital, Karamsad, following a series of complicated medical interventions after a challenging esophagectomy. The successful outcome highlights the expertise and collaborative efforts of the hospital's oncology and interventional pulmonology teams.

The patient, who initially presented with advanced esophageal cancer, first received chemotherapy to downstage the disease before undergoing an esophagectomy performed by a surgical oncologists Dr. Raghunandan G C and Dr. Sundaram Pillai at Manibhai Shivabhai Patel Cancer Centre of Shree Krishna Hospital. Post-surgery, on day three, the patient developed severe complications including a collapsed large left pneumothorax and a persistent air leak, necessitating his transfer to the Intensive Care Unit (ICU).

Despite the insertion of a second drainage tube and the application of a specialized three-bottle suction system, a subsequent HRCT scan revealed no improvement in his lung condition. To avert another major surgical procedure for a persistent Broncho-Pleural Fistula (BPF), interventional pulmonologists Dr. Abhishek Shah and Dr. Abhishek Prajapati performed a therapeutic flexible bronchoscopy. During the procedure, they first cleared a significant amount of thick mucopurulent secretions from the patient's airway.

Crucially, they located the air leak and sealed it using an Endobronchial Watanabe Spigot (EWS), followed by the intrabronchial installation of glue (ISO Amyl 2-cyanoacrylate) in the left lower lobe bronchus. Addressing a residual minor leak, they performed an Autologous Blood Patch Pleurodesis (ABPP) using the patient's own blood to further seal the leak.

The patient's recovery journey continued with additional challenges, including the need for another bronchoscopy to clear abundant thick mucopurulent secretions from his airway, with subsequent development of severe pneumonia and sepsis caused by Multi-Drug Resistant Gram-negative bacteria. He was managed in the ICU by the Critical Care team, led by Dr. Samir Patel, with aggressive broad-spectrum antibiotics therapy, coupled with continuous chest physiotherapy and pain management.

"The interventional Pulmonology team of Shree Krishna Hospital has now treated a significant number of Broncho-Pleural Fistula (BPF) due to various etiologies using therapeutic flexible bronchoscopy, preventing patients from undergoing complex and major surgery," stated the Pulmonologists.

"This case was particularly complex due to the advanced stage of the cancer. The multidisciplinary approach, innovative procedures, and unwavering dedication of our medical team were instrumental in achieving this positive outcome," stated the surgical oncologists.

After 22 days of intensive care and treatment, the patient was discharged in stable condition, marking a significant new lease of life for him and a testament to the advanced medical capabilities at Shree Krishna Hospital.
