1st -Gastrojejunostomy with LAMS in Kerala . First EUS-GJ with Hot Spaxus LAMS in Kerala
An 80/Female presented with symptomatic Gastric Outlet Obstruction (GOO). She had Intractable vomiting and an inability to tolerate oral intake secondary to Ampullary Carcinoma. She was previously treated with a Biliary SEMS in 2024 for Biliary obstruction, and now repeat CECT showed progression of disease with Duodenal Infiltration and Gastric Outlet Obstruction.
The plan was to do a minimally invasive bypass via Endoscopic Ultrasound-guided Gastrojejunostomy (EUS-GJ). We used a Niti-S Hot SPAXUS™ Lumen-Apposing Metal Stent (LAMS) and performed the 1st EUS-GJ using the electrocautery-enhanced Niti-S Hot SPAXUS™ Lumen-Apposing Metal Stent (LAMS) system in Kerala. 1st with in Kerala VPS Lakeshore Gastroenterology VPS Lakeshore
was first described for , a rare condition where the Oesophagus (the tube that connects the mouth to the stomach) has trouble moving food and liquids into the stomach. ( ) is a newer, minimally invasive treatment that involves using an endoscope (a flexible tube with a camera) to cut the muscles at the lower end of the esophagus. The first POEM was performed in 2008 by Inoue et al in Japan, and since then, it has been successfully used worldwide. We performed our 1st POEM in 2013. We use special electrosurgical knives for these procedures such as the TT-J knife or the Dual-J knife using standard Electrosurgical Generators. A newer advance in this field is the device using Newer technologies such as (Radio Frequency Ablation) as well as technology for Cutting, dissecting and coagulation. We had the opportunity to use the device for the 1st time in way back in December 2022 . We once again had the opportunity to use the updated device which is smaller and narrower but more efficient and easier to use for the 1st time in Kerala.
