🥄 From Tube to Taste – A Little Fighter’s Swallowing Journey 🥄

Three months ago, a 3-year-old girl with a traumatic brain injury was completely dependent on a nasogastric (NG) tube. She couldn’t swallow even a few drops of water safely.

Her oral phase was severely affected:
❌ Poor tongue movement
❌ Oral holding of food
❌ No safe swallow initiation
❌ Zero oral intake — everything was through the NG tube

But with intensive swallowing therapy, everything began to change.

Through weeks of structured oral motor exercises and carefully monitored oral trials, she is now able to:

Pill dysphagia is the difficulty or inability to swallow tablets or capsules. It can occur even if a person has no issues swallowing food or liquids.

Common Causes:

Fear or anxiety about choking

Dry mouth or reduced saliva

Aging (weakened throat muscles)

Neurological disorders (e.g., Parkinson’s, stroke, dementia)

Structural issues in the throat or esophagus

History of choking on pills

Risks:

Pills getting stuck in the throat or esophagus

Aspiration (pills or liquids entering the airway)

Reduced medication effectiveness

Avoidance of needed medications

- Common Challenges with Swallowing Pills

Pills can trigger gagging, get stuck, or cause choking.

Causes may be structural (tumors, surgery), physiological (Parkinson’s, ALS), or psychological (fear of swallowing).

Pills may lodge in the pharynx or esophagus, risking injury and ineffective medication delivery.

- Traditional Strategies (with Risks)

Liquid Medications – Not always available or safe for aspiration-prone individuals.

Embedding in Food – Risk of aspiration, reduced drug absorption, or rejection if the food is swallowed but pill is “pocketed.”

Crushing Pills – Many pills shouldn’t be crushed (e.g., time-release), and combining with thickened liquids or food can significantly reduce drug effectiveness (bioavailability).

Many people struggle to swallow pills due to physical, medical, or psychological reasons. Traditional methods like crushing pills or mixing them with food can reduce medication effectiveness or cause choking. Gloup, a pill swallow gel made from maltodextrin and seaweed extract, offers a safer and smoother way to swallow pills without affecting drug absorption. It comes in two consistencies (moderately thick and extremely thick) and has been clinically tested. While not suitable for all types of dysphagia, it’s a helpful solution for many, especially under guidance from a swallowing specialist.

Nasal regurgitation refers to the backward flow of nasal secretions into the throat. It can be caused by various factors such as nasal congestion, structural issues in the nose, or problems with the soft palate.

In a flexible endoscopic swallow study, nasal regurgitation may be observed when there is an abnormal passage of food or liquid into the nasal cavity during swallowing. This can be indicative of issues with the coordination of the swallowing mechanism or structural abnormalities.

Nasal regurgitation can have various causes, including:

Velopharyngeal Dysfunction: Poor closure of the soft palate (velopharyngeal dysfunction) can lead to nasal regurgitation, allowing food or liquids to enter the nasal cavity during swallowing.

Structural Issues: Abnormalities in the nasal passages or palate, such as cleft palate, can contribute to nasal regurgitation.

Neurological Conditions: Conditions affecting the nerves controlling swallowing, like neurological disorders or damage, may result in impaired coordination and nasal regurgitation.

Muscular Weakness: Weakness in the muscles involved in swallowing, including the soft palate muscles, can contribute to nasal regurgitation.

Post-Surgical Complications: Certain surgeries, especially those involving the palate or surrounding structures, can lead to nasal regurgitation as a complication.

Inflammatory Conditions: Inflammation in the nasal passages or throat can affect the normal functioning of these areas, leading to Regurgitation
