Pericardial Effusion: is the acute or chronic accumulation of fluid in the pericardial space (between the parietal and the visceral pericardium) and is often associated with a variety of underlying disorders. The fluid can be either bloody (e.g., following aortic dissection) or serous (usually idiopathic). As the pericardium is rather stiff, the capacity of the pericardial space is limited. In chronic effusion, the pericardium can stretch to a certain degree, accommodating slightly more fluid. In the acute setting, however, the added volume quickly exceeds the maximum capacity of the pericardial space. In both cases, the end result is often cardiac tamponade: compression of the heart that can lead to a life-threatening reduction in cardiac output. Pericardial effusion is initially asymptomatic, but cardiac tamponade has a distinct clinical presentation, including hypotension, tachycardia, jugular venous congestion, and pulsus paradoxus. Echocardiography is the most important diagnostic procedure and usually reveals an anechoic pericardial space. Treatment depends on hemodynamic stability: unstable patients require quick pericardial fluid drainage, through either pericardiocentesis or surgery, whereas in stable patients, treatment focuses on the underlying disease.

ACETYLSALICYLIC ACID : also known as aspirin, antiplatelet agent.

Mechanism of action:
Irreversible COX-1 inhibition β†’ inhibition of thromboxane (TXA2) synthesis in platelets β†’ inhibition of platelet aggregation (antithrombotic effect)

* Onset of antiplatelet action : within minutes
* Duration of antiplatelet action: 7 to 10 days

Effects :
* Low dose (below 300 mg/day): inhibition of platelet aggregation
* Intermediate dose (300-2400 mg/day): antipyretic and analgesic effect
* High dose (2400-4000 mg/day): antiinflammatory effect

Indication :
* Acute myocardial infarction
* Acute ischemic stroke
* Angina (stable and unstable)
* Management of ASCVD

Adverse effects:
* Dyspepsia
* Gastric ulceration, hemorrhage, perforation
* Increase bleeding time and prothrombin time
* Tinnitus
* Acute kidney injury
* Salicylate poisoning
* Allergic reaction

Contraindications:
* Febrile illness in individuals < 19 years of age
* Acute gout attack

πŸ§ͺLaboratory criterias for diagnosis of diabetes mellitus :

1) symptoms of diabetes + Random plasma glucose > 200 mg/dl, or
2) 8 hrs fasting plasma glucose > 126 mg/dl, or
3) 2 hrs post (after taking 75mg glucose ) plasma glucose> 200 mg/dl

Note:
* Impaired fasting glucose (IFG) : FBG levels b/w 110 and 125 mg/dl
* Impaired glucose tolerance (IGT) : Post glucose plasma levels b/w 141 and 199 mg/ dl
* Patients with IFG and IGT are not labelled as diabetics but are at risk of developing diabetes and cardiovascular disease.

Symptoms of diabetes :

1) Polyphagia, polydipsia, and polyuria
2) Unexplained weight loss
3) fatigue, Pruritus
4) Poor wound healing
5) Increased susceptibility to infections
6) Calf cramps

🧠 Diabetes mellitus should be suspected in patients with recurrent cellulitis, candidiasis, dermatophyte infections, gangrene, pneumonia (particularly tuberculosis reactivation), influenza, genitourinary infections (UTIs), osteomyelitis, and/or vascular dementia.
