We did yesterday a very interesting surgery.

A 51 years lady presented with chest pain in a city hospital. She was taken to cathlab to do coronary angiography which revealed triple vessel coronary artery disease and angioplasty was attempted. Unfortunately the balloon and the wire were stucked in the left main coronary artery and could not be retrived by catheter-based techniques.

She was referred to our hospital for CABG and open removal of the stucked balloon and wire.

We did CABG first on beating heart technique and then patient was briefly supported by cardiopulmonary bypass machine and heart was arrested and the aorta is opened to retrieve the balloon and wire on vision.

Patient is doing well now and would be discharged within 3 days.

So it's all well when it ends well.

MICS at its best.
For coronary artery bypass surgery a bench mark is total arterial revascularisation( bilateral mammary artery)
Advantages
1. Very long term patency of grafts , means you may not have a second heart attack or 2nd heart surgery in future because the arterial graft last lifelong in many patients
2. No cut in hand or foot to harvest artery or Vein
3. No touch in the aorta ( main artery arising from heart) , so chance of perioperative stroke is minimum
Previously this highly technically demanding operation was done only through sternotomy or opening the midline breast bone.
Now with advantage of advancing technology and skill we are doing total arterial revascularisation with LIMA - RIMA through MICS approach.
So
●no midline incision for cutting the breast bone,
●not a single cut mark in hand or feet
● advantages of LIMA- RIMA total arterial bypass through a small incision.
Definitely MICS CABG at its best.
Below is picture of a 64 year old gentleman ,who had undergone successful LIMA- RIMA TOTAL ARTERIAL BYPASS through a small incision on chest , without any other cut mark in hand and feet, today under our care.
This is our 50 th case of total arterial Minimally invasive CABG.

গৃহিণী, রোগী, স্বাভাবিক গৃহস্থালির কাজের সময় হঠাৎ ধড়ফড় এবং বুকে ব্যথা অনুভব করেন। রুটিন ইসিজি এবং ইকো ডপলার রোগ নির্ণয় নিশ্চিত করতে ব্যর্থ হন। মিসেস গীতার (নাম পরিবর্তিত) বিস্তারিত হার্ট স্ক্যান (সিটি করোনারি অ্যাঞ্জিওগ্রাফি) এবং তারপরে একটি বিশেষায়িত অ্যাঞ্জিওগ্রামে একটি বিরল এবং জটিল সমস্যা দেখা দেয়। রোগীর করোনারি ধমনী থেকে দুটি অস্বাভাবিক সংযোগ ছিল। একটি হৃদপিণ্ডের বাম প্রধান করোনারি ধমনী থেকে এসে তার ফুসফুসে যাওয়া প্রধান পালমোনারি আর্টারি ধমনীতে প্রবাহিত হচ্ছিল। দ্বিতীয়টি ডান করোনারি ধমনী থেকে একটি ছোট শাখা থেকে এসেছিল। এটি ফুসফুসের ধমনীর সাথেও সংযুক্ত ছিল। এই অতিরিক্ত, অস্বাভাবিক সংযোগের কারণে, রক্ত ​​স্বাভাবিকভাবে প্রবাহিত হচ্ছিল না। এর ফলে হৃদপিণ্ড আরও বেশি কাজ করছিল, এর পাম্পিং শক্তি হ্রাস পেয়েছিল এবং লক্ষণগুলি আরও খারাপ হতে শুরু করেছিল।

কনসালট্যান্ট কার্ডিওথোরাসিক সার্জন ডাঃ কৌশিক মুখার্জি ব্যাখ্যা করেছেন, “মিসেস গীতা তার একমাত্র ছেলেকে নিয়ে হাসপাতালে এসেছিলেন। তারা দুজনেই তার স্বাস্থ্যের অবস্থা নিয়ে খুব উদ্বিগ্ন এবং ভীত ছিলেন। আমাদের দল তাকে পরামর্শ দিয়েছিল এবং তার অবস্থা সমাধানের জন্য তাৎক্ষণিক পদক্ষেপ নিয়েছিল। আমরা রোগীর নিজস্ব প্রাকৃতিক হৃদপিণ্ডের আবরণ (পেরিকার্ডিয়াম) থেকে তৈরি একটি ছোট প্যাচ ব্যবহার করে অস্বাভাবিক সংযোগটি বন্ধ করে দিয়েছিলাম। অবাঞ্ছিত রক্ত ​​প্রবাহ বন্ধ করার জন্য আমরা ডান করোনারি ধমনীর ছোট ধমনী শাখাটি নিরাপদে বেঁধে দিয়েছিলাম। অস্ত্রোপচারের পর একই দিনে তাকে কৃত্রিম বায়ুচলাচলের বাইরে রাখা হয়েছিল।”

মাত্র চার দিন পর তিনি বাড়ি ফিরে যান এবং আশা করা হচ্ছে যে তিনি অতিরিক্ত হৃদরোগের ঝুঁকি ছাড়াই স্বাভাবিক, সুস্থ জীবনযাপন করবেন।

Kolkata : A 56-year-old lady from Diamond Harbour was successfully treated for a rare and potentially serious heart condition known as Coronary Artery Fistula (CAF), an anomaly seen in only about 0.02% of the general population. The expert team led by Dr Kaushik Mukherjee, Consultant Cardiothoracic Surgeon, Manipal Hospital Dhakuria completed the surgery in 2.5 hours.

The patient, a home maker , experienced breathing difficulties during normal household work with sudden palpitation and chest pain. Routine ECG and ECHO doppler failed to establish a diagnosis. A detailed heart scan (CT coronary angiography) of Mrs Geeta (name changed) followed by a specialized angiogram showed a rare and complicated problem. The patient had two abnormal connections from her coronary arteries. One was coming from the left main coronary artery of the heart and draining into the main Pulmonary Artery going to her lungs. The second was from a small branch from the right coronary artery. also connecting to the lung artery. Because of these extra, abnormal connections, blood was not flowing in the normal way. This made the heart work harder, reduced its pumping strength, and led to worsening symptoms.

While many patients remain asymptomatic early in life, nearly 50% develop symptoms in adulthood. The condition can lead to reduced blood supply to the heart muscle, a phenomenon known as coronary steal syndrome causing breathlessness, chest pain, unstable angina, and in rare cases, heart attack.

Dr Kaushik Mukherjee, Consultant Cardiothoracic Surgeon, explained, “ Mrs Geeta came to the hospital accompanied by her only son . Both of them were very anxious and frightened about her health condition. Our team counselled her and took immediate steps to resolve her condition. We closed the abnormal connection using a small patch made from the patient’s own natural heart covering ( pericardium). We also safely tied off the small artery branch from right coronary artery to stop the unwanted blood flow. She was out of artificial ventilation in on the same day after surgery.”

Her recovery was uneventful and she went home just four days later and is expected to live a normal, healthy life without extra heart risk.
