Acute Adrenal Cortical Insufficiency

(Adrenal Crisis, Waterhouse–Friderichsen Syndrome)

Cause:
• Sudden bleeding and tissue death in the adrenal cortex
• Most often from severe bacterial infection in the blood (septicemia), especially:
• Meningococcal septicemia (most common)
• Pseudomonas, pneumococci, Haemophilus influenzae
• Bacterial toxins damage blood vessels → widespread clotting & bleeding → adrenal destruction
• Other causes: birth trauma, anticoagulant use, severe DIC

Symptoms:
• Severe low blood pressure (shock) not improving with fluids
• Sudden weakness and extreme fatigue
• Confusion, drowsiness, or coma
• High fever (often with infection)
• Nausea, vomiting, abdominal pain
• Low blood sugar (hypoglycemia)
• Darkening of the skin (sometimes)

Management:
1. Immediate emergency treatment – do not delay for tests
2. Give IV hydrocortisone immediately
3. Rapid IV fluids (normal saline ± glucose) to correct shock & low blood sugar
4. Treat the underlying infection with broad-spectrum IV antibiotics
5. Correct electrolyte imbalances (low sodium, high potassium)
6. Monitor vital signs, glucose, and electrolytes closely
