SINGLE STAGE BILATERAL TOTAL HIP REPLACEMENT

Reminiscing the awareness programmes and celebrations of World Arthritis Day, 12th October, we share a case of a young lady with advanced osteoarthritis of both the hip joints, crippling her activities of daily living.

To perform a Total Hip Replacement surgery of both the hips in a single sitting needs a state of the art infrastructure, expert operative room personnel, great anaesthetic team back up, and most importantly a suitable patient.

This was probably the first case of bilateral hip replacement done in the same sitting in North Bengal, and we are glad to restore the patient back to her normal life.

A scientific forum for invited speakers to present their original work for education.

This year I had the honour and privilege to present a paper and give a talk on posterolateral corner injuries of the knee as well as an e-poster demonstration of the use of 3D printing in complex hip arthroplasties.

Great academic extravaganza and camaraderie.

|| HUMANOID ROBOTIC ASSISTED TKR ||

Earlier this month, North Bengal witnessed history by having its first ever Humanoid Robotic Assisted Total Knee Replacement.

The Microsoft enabled AI powered hololens enables the ex*****on of a Total Knee Replacement with utmost precision, reducing human error and hence enhancing the outcome. With the help of this technology we develop a CT based planning to devise a patient specific instrumentation for the best implantation and outcome.

We operated a 70 year old gentleman with this technique, and the success spoke for itself when the patient himself joined in for the press conference -

This opens the horizon for joint replacement surgeries in North Bengal and its allied areas.

|| MEGA PROSTHESIS ||

A mega prosthesis is a large, modular prosthetic implant system used for complex knee reconstructions, particularly after tumor resections or in cases of severe bone loss secondary to fracture or infections.

This 60 year old man presented to us with a history of surgery done for his right distal femur fracture elsewhere about one year back. He had not been able to walk or bear any sort of weight following surgery.

Radiographic evaluation (no previous xrays were there) revealed a failed fixation of distal femur with intra-articular screws, posterior condyles lying separately, and an extensively damaged joint surface.

After ruling out infection, we planned for a single stage revision. Joint salvage was not an option and considering the extent of bone loss in the femur, a mega prosthesis was used for articular reconstruction.

The patient was allowed to walk immediately the next day onwards.

Ps: Arthrodesis was not an option as the patient had bilateral foot drop from a previous spine surgery, hence needed a functional joint for adequate ground clearance.

This also happened to be the FIRST USE of MEGA PROSTHESIS in a trauma scenario in North Bengal.

Honoured to be invited as a national faculty for this two day course, over two states. Extensive discussion on total hip arthroplasty with presentations. Had the privilege of presenting my own complex primary total hip arthroplasty cases with acetabular bone defects and also femoral deformities. Good participation, great academics and hospitality.

Odontoid fracture | C1-C2 fusion

The odontoid is a projection from the second cervical vertebrae (axis) on which the first cervical vertebrae (atlas) pivots.

Life-threatening fractures can occur as a result of forces acting on this structure, due to its proximity to the spinal cord and brainstem.

We had this 29 years old unfortunate civil engineer who fell off from the third storey of one of his construction sites in Guwahati. He was airlifted to us, and we received him in a state of quadriparesis (weakness of both the upper and lower limbs).

C1 - C2 fusion was done for the patient. He also had a minimally displaced iliac blade fracture and a patella fracture, which were conserved.

Even though his rehab took time considering the associated fractures, it was great to see him come back for his third month follow up with almost complete recovery.
