Story and medical course of an extremely high risk patient who develops rare of the rarest complication following gallbladder stone surgery
this patient initially underwent laparoscopic surgery for gallbladder stone during which she developed an injury to the bile duct. Following this she was initially managed with ERCP and antibiotics but developed internal bile and collection with sepsis.she was being managed conservatively for these problems however she developed a pseudoaneurysm from the right hepatic artery from which she started bleeding repeatedly.
Her main blood supply to the liver consisting of the portal Vein was surprisingly thrombosed and blocked. she underwent an initial angiography and embolisation but it failed to tackle the aneurysm and the bleeding persisted. She also started having sepsis and infection on account of the associated bile leak. Her albumin level fell to 2 g/dl.
she was extremely high-risk for surgery with a very high risk for liver failure and death and hence was refused initial surgery by the medical board. however considering the fact that she was going to die any way, after due counselling and with extremely high risk she was operated by Dr Sanjay Mandal. The surgery was extremely difficult and took five hours, the bleeding vessel and aneurysm could be successful ligated .
she had a very difficult post-operative Recovery, however she could be saved and sent home in a stable condition.

this young homemaker initially had presented with severe necrotising Gall Stone pancreatitis for which she required prolong hospitalisation and ICU stay. she was subsequently discharged, however on account of the lingering pancreatitis she developed a huge pseudocyst of the pancreas 20cm x 20cm x 15cm which not only started causing severe pain abdomen but also by compressing on the stomach was preventing her from taking food. She started developing severe weight loss.
She underwent a successful laparoscopic cystogastrostomy and cholecystectomy, in which the cyst was drained into the stomach by creating a special bypass thereby relieving the symptoms.

This interesting case of a young lady who presented with symptoms of gradually increasing abdominal distension for the past few months. Her relatives initially thought that she was pregnant, however when she visited a doctor the true cause of the distension was found out to be a huge tumour in the spleen.
this tumour was in a very crucial area pressing on the pancreas, stomach and the large intestine. It was also involving the left side of the diaphragm.
She underwent a 4 hour surgery to remove the Tumor arising from the spleen which turned out to be 35 cm X 30 cm X 25 cm and weighed almost 6 kg.
