Let's learn about an important procedure called cervical cerclage, which can help prevent miscarriage in selected high-risk pregnancies.

Cervical cerclage is a procedure where we place a stitch around the cervix to keep it closed during pregnancy. It is usually recommended in cases of a weak cervix, also known as cervical insufficiency, where the cervix may open early without pain.

We advise cerclage in situations like
• previous second trimester miscarriages
• short cervix on ultrasound
• early opening of the cervix

This stitch helps support the pregnancy, prolong it, and reduce the risk of preterm birth, giving the baby a safer environment to grow.

In this reel, I am showing a transvaginal ultrasound scan of a patient with completely normal findings, to help you understand what a healthy uterus looks like.
The endometrium is the inner lining of the uterus where the embryo implants. On a transvaginal ultrasound, we assess the endometrium based on three key factors
its thickness, its pattern (especially the triple line appearance), and its blood flow.

A well-defined triple line pattern with adequate thickness usually indicates that the lining is receptive and ready for implantation.
We also look at the ovaries, where follicles develop. On ultrasound, normal ovaries show developing follicles and a healthy structure, which reflects good ovarian function.

This scan is a good example of how a normal endometrium and normal ovaries appear on ultrasound.

Understanding these basics helps patients feel more aware and confident about their fertility journey.

During a recent case discussion with my embryologist, we were reviewing a patient’s semen analysis report and noticed increased leucocyte levels.

Leucocytes are white blood cells, and when they appear in higher numbers in semen, it may indicate an underlying infection or inflammation in the reproductive tract. This condition is often referred to as leukocytospermia.

In many cases, the cause can be a mild infection that responds well to proper hydration and a course of antibiotics. Sometimes lifestyle factors like smoking or alcohol consumption, or medical conditions such as diabetes, can also contribute to increased leucocytes.

To understand the exact cause, we may recommend further evaluation such as a semen culture, which helps identify any infection and guide the right treatment.

The good news is that in many patients, a simple and timely treatment significantly improves the semen parameters.
In fertility treatment, even small findings in reports deserve attention.

Addressing them early can make a meaningful difference in outcomes.

हाल ही में मैं अपने एम्ब्रायोलॉजिस्ट के साथ एक केस पर चर्चा कर रही थी, जिसमें सीमन एनालिसिस रिपोर्ट में ल्यूकोसाइट्स (white blood cells) की मात्रा बढ़ी हुई पाई गई।

जब सीमन में ल्यूकोसाइट्स अधिक होते हैं, तो यह अक्सर किसी इन्फेक्शन या सूजन का संकेत हो सकता है। कई मामलों में यह समस्या सही हाइड्रेशन और एंटीबायोटिक कोर्स से ठीक हो जाती है।

कभी-कभी स्मोकिंग, अल्कोहल या डायबिटीज जैसी मेडिकल कंडीशन भी इसके पीछे कारण हो सकती है।

ऐसे में हम सीमन कल्चर की सलाह भी देते हैं, जिससे इन्फेक्शन की सही पहचान कर उचित इलाज किया जा सके।
अच्छी बात यह है कि समय पर सही इलाज से यह समस्या अक्सर ठीक हो जाती है और रिपोर्ट में सुधार देखने को मिलता है।

फर्टिलिटी ट्रीटमेंट में छोटी-छोटी बातें भी बहुत मायने रखती हैं। सही समय पर ध्यान देना ही बेहतर परिणाम की कुंजी है।

One of my patients once came to me very worried right after her embryo transfer. She said, “Doctor, now I will not walk, cough or even sneeze. What if the embryo falls off?”

This is a very common myth I hear from IVF patients.

Let me clarify. The embryo that we transfer is extremely tiny. It cannot be seen with the naked eye and is visible only under a microscope. Once it is placed inside the uterus, it does not simply “fall out” because of normal activities.

Your uterus is designed to hold and protect the embryo. After the transfer, the embryo settles in the uterine lining and begins the natural process of implantation.

So coughing, sneezing, walking or doing routine daily activities will not dislodge the embryo.

Of course, I advise avoiding heavy physical strain and following your medication schedule, but normal movements are absolutely safe.

IVF treatment already brings a lot of anxiety for couples. My job is not only to treat but also to clear these myths so that patients can stay calm and confident during their journey.

Trust science, not rumors.

In one of my recent consultations, a patient called me in panic saying she was bleeding during early pregnancy. Bleeding in pregnancy can be frightening, so we did an immediate ultrasound.

Her scan showed a condition called subchorionic hemorrhage. This happens when a small amount of blood collects between the gestational sac and the uterine wall. It is one of the more common causes of bleeding in the first trimester. In many cases, this bleeding settles on its own, but careful monitoring is important because it can increase the risk of complications if left unchecked.

Subchorionic hemorrhage may cause spotting or light bleeding, and while it doesn’t always lead to problems, close medical supervision helps us ensure the pregnancy continues safely.
We treated her supportively, advised rest and careful observation, and monitored her for five days.

Gradually the bleeding resolved, and she went on to be blessed with a beautiful, healthy baby.

The message is simple: bleeding in early pregnancy doesn’t always mean danger, but it should never be ignored. Timely evaluation, proper care, and calm monitoring make all the difference.

कभी-कभी गर्भावस्था के शुरुआती महीनों में अचानक bleeding या spotting देखकर मरीज बहुत घबरा जाते हैं। मेरे पास भी एक मरीज का फोन आया जो panic में थी और कह रही थी कि bleeding हो रही है।

Ultrasound जांच में पता चला कि उन्हें Subchorionic Hemorrhage हुआ है। यह एक ऐसी स्थिति होती है जिसमें गर्भ थैली और गर्भाशय की दीवार के बीच थोड़ा खून जमा हो जाता है।

यह शुरुआती pregnancy में bleeding का एक सामान्य कारण माना जाता है और कई मामलों में सही देखभाल से अपने-आप ठीक भी हो जाता है।

इसके लक्षण spotting या हल्की bleeding हो सकते हैं। अधिकतर छोटे hematoma समय के साथ absorb हो जाते हैं, लेकिन डॉक्टर की निगरानी जरूरी होती है क्योंकि कुछ मामलों में miscarriage या अन्य complications का जोखिम बढ़ सकता है।

मैंने मरीज को दवाइयाँ दीं, आराम की सलाह दी और करीब 5 दिनों तक लगातार monitoring की। धीरे-धीरे bleeding बंद हो गई और pregnancy सुरक्षित रही।
आज वही मरीज एक स्वस्थ, प्यारे बच्चे की माँ है।

Understanding the uterus is the first step towards understanding fertility.
In this reel, I explain what an anteverted uterus looks like on ultrasound and why the uterine lining plays such an important role in IVF success.

We look at the layers of the uterus, the myometrium where fibroids may develop, and the endometrium, the lining where implantation happens. A healthy endometrium often shows a clear triple line pattern, which tells us the uterus is preparing well for pregnancy.

Along with thickness, blood flow is equally important. Using colour Doppler, we assess how deeply blood supply is reaching the lining. When blood flow reaches the inner zones, especially zone 4, the chances of implantation improve.

During an FET cycle, embryos are already frozen and grown in the lab. Before transferring them, my focus is on preparing the uterine lining properly checking thickness, triple line appearance and blood flow.

Because successful IVF is not just about creating good embryos. It is about placing them in the right environment at the right time.

फर्टिलिटी को समझने की शुरुआत गर्भाशय को समझने से होती है।

इस रील में मैं अल्ट्रासाउंड के माध्यम से एंटिवर्टेड यूटेरस की संरचना समझा रही हूँ और बता रही हूँ कि IVF की सफलता में गर्भाशय की लाइनिंग कितनी महत्वपूर्ण भूमिका निभाती है।
हम यूटेरस की लेयर्स को देखते हैं, मायोमेट्रियम, जहाँ फाइब्रॉइड्स बन सकते हैं, और एंडोमेट्रियम, यानी वह लाइनिंग जहाँ भ्रूण का इम्प्लांटेशन होता है। एक स्वस्थ एंडोमेट्रियम में अक्सर स्पष्ट ट्रिपल लाइन पैटर्न दिखाई देता है, जो इस बात का संकेत है कि यूटेरस प्रेग्नेंसी के लिए तैयार हो रहा है।

सिर्फ मोटाई ही नहीं, ब्लड फ्लो भी उतना ही जरूरी है। कलर डॉप्लर की मदद से हम देखते हैं कि रक्त प्रवाह लाइनिंग के अंदर तक कितनी गहराई तक पहुँच रहा है। जब ब्लड फ्लो अंदरूनी ज़ोन, खासकर ज़ोन 4 तक पहुँचता है, तो इम्प्लांटेशन के चांस बेहतर हो जाते हैं।

FET साइकिल में एम्ब्रियो पहले से लैब में तैयार और फ्रीज़ होते हैं। ट्रांसफर से पहले मेरा पूरा ध्यान यूटेरस की लाइनिंग को सही तरीके से तैयार करने पर होता है — उसकी मोटाई, ट्रिपल लाइन और ब्लड फ्लो की जाँच पर।

क्योंकि सफल IVF सिर्फ अच्छे एम्ब्रियो बनाने से नहीं होता, बल्कि उन्हें सही समय पर सही वातावरण देने से होता है।
