History :
I have planned and placed ( ALL ON 4 concept) Nobel active implants. Four implants were surgically placed in upper jaw and three implants in lower jaw in 33 35 and 45 region.
There was a huge pathology / Granulatous Tissue and infected bone ( peri apical cyst ) in region of 42 43 44 region. So it was cleaned and grafted.
After 6 months, it was observed that graft / membrane didn't worked and it was a set back to see it diagnostically & clinically.
Present scenario :
It was decided to redo the Grafting with implant Placement.
Keeping the complete guidelines and Infection control protocol under Local anaesthesia, flap was surgically raised.
The previous graft and all the infected tissue was removed.
Now the questions arises if again graft doesn't work then what should be the alternative .
So it was decided to place 2 Nobel active implant's , one in 41 and other at 43 region. ( so total of 5 implants in lower jaw), and also strengthening the lower fixed prosthesis in future.
3.5 / 11mm in 41 region and 4.3/ 10mm in 43 region Nobel Active implants were surgically placed.
BioOss Graft and BIO gide resorbable bilayer membrane was used. Cytoplast sutured were then placed.

Patient was advised to discontinue lower denture. (which was supported by 3 dental implants earlier )
The surgery was performed by well renowned Oral Implantologist and our mentor Dr Lanka Mahesh .
