A young patient landed in emergency with severe colicky pain on Left side, a classic presentation of ureteric colic. CT scan revealed a small stone in left lower ureter and a large stone in right kidney with surprise finding of horseshoe kidney!
Horseshoe kidney is an anatomical variant found in 1 in 400 people. Here, the lower poles of the two kidneys are fused to give a horseshoe like appearance. And in half of people, it remains asymptomatic. However, due to anatomic abnormality, these kidneys are more prone to stone formation.
As we informed the patient about our findings and plan to remove this big stone in the same sitting, his response “Yes I know doctor. Two other doctors had attempted to remove this stone, but have failed. You can also try!” took us aback! The patient told us that at first time, RIRS was attempted. On visualising the stone, the urologist realised that the stone bulk was too large to give a reasonable clearance and aborted the procedure. The next time, PCNL was attempted, but the initial access could not be achieved and again, the procedure was aborted.
The right part of kidney had two stones, a 2 cm and a 1 cm stone. Dealing with such stones requires a thorough knowledge about anatomical variation as well as choosing the right approach / procedure for optimal outcomes. RIRS is an excellent modality for managing kidney stones in a scar-less way, and is a boon for patients on blood thinners / anatomical variations. However, stone clearance rates go down as the stone bulk (volume) increases. Larger sized stones require multiple sittings. PCNL on the other hand, is technically demanding procedure for larger stone bulk. In experienced hands, it gives excellent clearance rates. A 3D understanding of anatomy is a must for successful procedure. Patience of surgeon is pre-requisite for achieving low complication rates.
In this patient, after detailed discussion regarding his anatomical variation and proposed procedure for stone removal was done. All his queries were answered. He finally underwent Right PCNL with DJ stenting. Superior calyceal access was done and complete clearance of stone was achieved. Before scheduling DJ stent removal, a check CT scan confirmed complete stone clearance. Needless to say, the patient was happy to get rid of the unwanted guests out of his kidneys. And, the satisfaction of being able to help yet another patient, simply priceless!

The doctor’s waiting time is too much!
The other day, as I completed consultation of one patient and asked for the next one, he was nowhere to be seen. My OPD co-ordinator informed me that the patient got irritated that he was waiting for too long and has left!
It cannot be denied that time is a very precious currency for all and we all expect punctuality in all our appointments. But, can we doctors really predict how much time will a particular patient need in a consultation?
A seemingly straightforward case can be deceptively complicated. One patient comes with a textbook presentation and straightforward diagnosis, and another one with a complicated history spanning a few years! There is a simple infection needing simple explanation and a small prescription; then, there is a devastating diagnosis of cancer requiring time to break the news, let the patient/attendants absorb the news and then counsel them regarding treatment plan, the expected outcomes and life expectancy with all patience and empathy to provide them with assurance and comfort. Any surgical plan needs a detailed discussion regarding all advantages, disadvantages, possible outcomes including complications, which becomes even more important with cases like kidney transplantation or surgery for cancer. A follow up after successful surgical procedure needs few minutes, whereas, a post-surgical complication needs unlimited time and patience to assure them that we are with them, and will help them to overcome this difficult phase of treatment. And, all pertinent points are to be documented and prescription is explained as in contemporary practice, all patients are potential litigants!
And, if one is a surgical specialist, one also needs juggle between OPDs and Operating Rooms. The time required in a surgery is as unpredictable as the life itself! We can land up in trouble when its least expected!! But, whatever may happen, no surgeon rushes through the surgery.
And, most of the doctors try their level best to attend their OPDs in a timely fashion.
Sometimes, I wonder, people can patiently stand up in line without complaining when it comes to getting hold of new iPhone, or tickets for a concert. But, are extremely impatient when waiting outside clinic. Why can’t we understand that the previous patient may be discussing some life altering situation with his / her physician? And, maybe, is in a much worse condition than you!
