During the recovery period post-surgery, the knee remains painful as it is healing back to normal. Generally, it takes ๐Ÿ‘-๐Ÿ” ๐ฆ๐จ๐ง๐ญ๐ก๐ฌ to complete recovery and to get a benefit out of the surgery.
Appropriate activity modifications are required to make the knee implant last for many years.

The patient in this video had stroke 8 months back, resulting in left side weakness, since then he is on an aggressive physiotherapy treatment. This patient started his road to recovery soon after stroke.

He was put on trademill training right after we achieved
1) good sitting balance
1) good standing balance
2) Ability to walk independently with a walking aid
3) Able to understand commands
4) stable cardiac status

We would have excluded him if he had anyother co-morbidities which would have not supported him to do trademill walking or if he had unstable cardiac status.

Post trademill walking what has improved:
1) His walking speed has increased.
2) His standing balance and endurance
3) Agility and co-ordination has improved
4) Boost in his energy
5) Quality of walking pattern was improved.

Trademill training has been widely used to improve walking ability post stroke. However, one has to consult his/her Physiotherapist for providing sufficient and appropriate details on when to start.

Current status of this patient: He walks using one stick and is happily going to his office in Andheri.

This is usually the chronology seen in frozen shoulder. An individual starts feeling pain due to which shoulder movement is restricted. This leads to inactivity and further restriction of movement.
Soon one realizes reaching over top shelf or performing daily activity has become difficult & now everybody tells youโ€ฆOh!! You have developed FROZEN SHOULDER.

Your shoulder joint is enclosed in a capsule made up of connective tissue. When this capsule thickens and tightens around the shoulder joint, the shoulder movements are restricted.
The exact cause is unknown. But people having diabetes, systemic conditions, or who had to recently immobilize the shoulder are at high risk of developing frozen shoulder.

Physiotherapy includes exercise therapy and electrical modalities for pain management. Studies have shown when NSAIDs and exercises are used together, the recovery rate is better.
Due to inactivity, deconditioning of shoulder and scapular muscles are commonly seen. Therefore strengthening remains an important part of physical therapy treatment.
Without the inclusion of physiotherapy recovery of movement is prolonged, imbalanced postures are developed, muscle deconditioning is seen.

๐™๐™ž๐™ฅ๐™จ ๐™ฉ๐™ค ๐™˜๐™ค๐™ข๐™—๐™–๐™ฉ ๐™๐™ง๐™ค๐™ฏ๐™š๐™ฃ ๐™Ž๐™๐™ค๐™ช๐™ก๐™™๐™š๐™ง
โ€ข Exercises should be gradually added taking into consideration stages of frozen shoulder.
โ€ข Aggressive approach in earlier stages may aggravate the synovitis and inflammation. Similarly, an easy approach in later stages may hinder the recovery process. Use a graded and guided approach.
โ€ข Blood sugar levels should be monitored and managed regularly. Any systemic conditions to be taken regular follow-ups with their doctors
โ€ข Patient education essential to reduce frustration and encourage compliance. Appropriate home program to be given which is easy to comply with as daily exercise is critical in relieving symptoms.

According to WHO, 15percent of the world population are living with disability.This year 2020 has brought up many anxieties issues in us. We're still living through uncertainty amidst a global public health crisis.

During this pandemic, many individuals have suffered isolation, loss of their loved ones, emotional and financial instabilities. Many were unable to access the support then let it be medical help or family support.

A huge part of our population suffers evident physical challenges which should not go unrecognized either. Its necessary for them to be included and make them a part of our soceity. This day lets join hands to recognise the needs of those whose disabilities are visible or invisible and are suffering. Lets be aware and spread awareness about their rights. Additionally, be empathetic and make this soceity an inclusive one.

LOW BACK PAIN (Part 2)

There is no one standard treatment plan in terms of physiotherapy for low back pain. Every condition is different and it varies with every individual.
Following is an outline of general treatment plan which should be followed. Also know about some preventive measures to avoid the pain.

Treatment:

Acute back pain: Usually gets better on its own. Treated with medications, heat/ice/gentle stretching.
Exercising, continuous bed rest and surgery are typically not recommended for acute low back pain. If recurrence rate is higher, consultation from a medical expert highly recommended.

Chronic back pain: Often treated with stepped care approach. Moving from simple aggressive approaches. Depends on cause and severity of back pain.

Step 1: (Early treatment)
Medications
Hot / Cold packs
Exercises

Step 2: (Alternative technique)
Physical therapy
Spinal injections
Accupuncture

Step 3: (Advanced Care)
This option may be considered after careful study of persons complaints and compliance to other treatment approaches.

Prevention Strategies:
โœ“Exercise regularly: Keep your muscles strong and flexible. Consult your physiotherapist for a low impact, age appropriate exercises that are specifically targeted to strengthening lower back muscles and abdominal.

โœ“Obesity and Diet: Weight Management and Healthy diet will help promote new bone growth and reduce extra stress over joints and muscles

โœ“Ergonomics: Use ergonomically designed furniture and equipments at home/work

โœ“Body mechanics: Improper body mechanics may be prevented by avoiding movements that jolt or strain the back.

โœ“Quit smoking: Smoking causes reduced blood supply to lower spinal segments causing early degeneration of disc. Also increases risk of osteoporosis and restricts healing. Coughing due to heavy smoking may cause back pain.

๐—”๐—ด๐—ฒ:Typically occurs in age group of 30-50 years. More common as age advances. As age increases there is loss of bone strength, reduced muscle elasticity and muscle tone which can lead to fracture thus causing back pain.

๐—ข๐—ฏ๐—ฒ๐˜€๐—ถ๐˜๐˜†:Sudden increase in weight, gradually increasing obesity leads to increased stress on low back causing pain

๐—™๐—ถ๐˜๐—ป๐—ฒ๐˜€๐˜€ ๐—น๐—ฒ๐˜ƒ๐—ฒ๐—น:More common who are not physically fit or active. Weak back and abdominal muscles may not properly support the spine. Also lot of exercise after a long period of inactivity, improper designed exercise regimes can cause injuries to back.
