“Teachers affect an eternity; they can never tell where their influence stops.” - Henry Brooks Adams.

As you grow old, you realise that learning never ends; and as you look back, you feel indebted to the Gurus who mould your life in one way or the other.

When I look back, and think about my Gurus; I feel grateful for all of them - my teachers, my parents, my mentors, guiding lights, siblings, some friends, some well wishers, some seniors, some juniors, some strangers, some acquaintances, even some cold shoulders. Thank you for the lessons and experiences.

They say "Let bygones be bygones" but last week was special and it deserves a mention in the story that's unfolding. I was about to leave the hospital after wrapping up my work when I received an exceptional invitation and a few days later we were witnessing the opulence and magnanimity that hallmarked the wedding of the decade.
Anant and Radhika's wedding was a beautiful celebration filled with joy and tradition. The venue was adorned with vibrant flowers and elegant decorations. Attention to detail was flawless and the venue was themed around Anant's love for animals. Guests rejoiced with music and dancing, savoring delicious cuisine throughout the festivities. Anant and Radhika exchanged vows amidst love and laughter, marking the beginning of their new journey together, embraced by the warmth of their loved ones. I must add, Mr Mukesh Ambani and Mrs Nita Ambani are such graceful and grounded human beings. They held the stage for almost 7 to 8 hours to ensure that they meet everyone who was there to bless the couple. An unforgettable memory to mark the union of two souls. Wishing them the best !

Cancer doesn't have to define you !!
Though it's one of the greatest maladies faced by the patient and its treatment is one of the greatest challenges faced by the clinician
- "There's always hope beyond what you see"

Sharing immidiate post operative pictures of success stories of one such patient who came to us at Max superspeciality hospital, Saket, New Delhi - having a KIDNEY CANCER WITH A VENOUS THROMBUS

I remember talking to this patient just before I was going to operate upon him. He asked a very specific question -

"Sir, this cancer has spread to the veins, would this surgery help me ? How long do I have ?"

(He must have googled something, I guess)

I remember reassuring him that things are not as bleak as he fears, most of his anxiety was based on half baked knowledge that he got from the internet; and we have an excellent chance of curing him with this minimally invasive surgery (key hole surgery / laparoscopy) in which the affected kidney as well as the Venous thrombus could be removed laparoscopically. He said thank you before the drugs kicked in and he was put under anesthesia.

Venous migration and tumor thrombus formation are unique aspects of renal cancers. It has been reported to occur in 4%–10% of patients with kidney cancers.

Surgical objectives while managing such patients are
1) Complete resection of tumor and the thrombi
2) Prevention of tumor embolism
3) minimizing blood loss
4) maintenance of hemodynamic stability
5) prevention of vital organ ischemia.

By grace of the almighty and the efforts of the team, the intended objectives were achieved. The patient is discharged and doing well.

He understands the importance of follow up and the crucial role it plays in cancer treatment.

He still messages me at times whenever he has some queries and I am happy answering them because I know he's not getting any half baked information from the internet anymore.

Every life you touch, changes you for the better. I believe as surgeons, as oncologists, as urologists, as clinicians; we should give the confidence to our patients that they feel free to connect to us. It helps them fight, stay positive and to emerge victorious, as SURVIVORS of this ruthless disease

Stepping on a Landmine !!

A pheochromocytoma is a hormone secreting tumor that develops in the adrenal gland.

You have two adrenal glands — one located at the top of each kidney. The adrenal glands are part of the body's hormone-producing (endocrine) system.

If you have a pheochromocytoma, the tumor releases hormones that may cause high blood pressure, headache, sweating and symptoms of a panic attack.

If a pheochromocytoma isn't treated, severe or life-threatening damage to other body systems can result.

This patient presented to us with such symptoms and was diagnosed with Pheochromocytoma.

Unlike other surgeries, adrenal removal for pheochromocytoma requires immaculate preoperative preparation to avoid intraoperative rise in blood pressure. The patient was well prepared and planned for surgery.

Robotic assisted Adrenalectomy was done and the patient is doing well.

Intraoperative parameters were well managed by the anesthesia team.

Tips and tricks -

1) No tumor handling before clipping/ ligation of the adrenal vein

2) Dissection of adrenal gland from the kidney is a crucial step. Caution is advised to avoid injuring the arterial branches to the upper pole of kidney.

3) One can encounter multiple vessels feeding the tumor. Careful clipping and division of these vessels would prevent intraoperative blood loss and unexpected rise in blood pressure

4) Communication with the anesthesia team is critical before ligation of the adrenal vein and tumor handling as well as after removal of the tumor.

You don't step on the Landmine if you keep these things in mind

Life is a journey and we are all connected. I remember reading "You can win" by Shri Shiv Khera jee while I was still in school. Today the 'reader-author' relationship was overshadowed by 'Doctor-Patient' relationship. I almost jumped like a teenager upon seeing him at the office today.

Laparascopic Nephrectomy for Non Functioning Kidney.

A humble request to kidney stone patients - Please don't ignore your stones. A neglected stone can lead to non functioning kidney. Stitchless and keyhole surgical options are available for treatment of kidney stones.

This patient had a symptomatic left non functioning kidney due to a neglected stone. The kidney was removed by laparoscopic surgery (four 1 cm incisions) and the patient was benefitted by key hole surgery, further complications as well as big scars were avoided.
