Again something that resonates with GOOD QUALITY SLEEP AND CHRONIC PAIN-

Again i came across an article in news that how to front line soldiers sleep during an high stress enviornment - it cannot get more stressed when ur in 1st line of defence for country in an Hot zone-

This lead me to thinking dont patients with chronic pain - as in cases of TMD and Orofacial Pain - suffer the same.

And coincidently they 90% of chronnic pain patients suffer from poor sleep quality - the above mentioned technique is exactly what i used to suggest -
Concentrate on breathing, slow deep breaths, realx and imagine each muscle that u know of from head to toe relaxing - with good sleep hygiene.
Amd surprisngly this technique works and many patients have given a positive feed back!!

TMJ Locking in News!!

Kudos to the primary care provider at railways platform who exactly shows how to reduce "Locked Jaw".

Increasingly many patients are being suffering from TMJ disorders, specifically affecting joint.

A lesser know issue but now becoming increasing commen where patients are faced by 1 of 2 conditions by -

Either cant open completley wide - Known as Closed lock
Cannot close as in case above - known as open lock.

Its an emergency in sense, but not life threatening and needs immediate attention to easily reduce before muscles undergo spasm!!

Challenges faced -
1.multiple non restorable teeth
2.Cross bite anterior n posterior
3.Narrow arch palate
4.Fragile enamel and dentine
5.Pt getting married in March 2026.

Face represents the largest cortical expression on the brain covering nearly 35 to 40% - Homonculus.

Hence the face which is richly supplied by more than 6 cranial nerves, with bit of C1C2 division makes diagnosing the facial pain, its source and referral pattern difficult.

Sneek Peek into the TMJ Capsule!!

Very few Drs treating TMDs have actually visualised the joint intra capsularly -

Being fortunate to assist Dr Tofiq Bohra, who is a pioneer in Level 3 Arthroscopy in India, and being associated with him for about 100s of TMJ cases - which has lead me to realise that not all cases can be treated conservatively- And NO MRI can conclusively capture the damage done to the articular disc and retrodiscal tissues!!

Always while evaluating patients, we need have a mental picture of the joint, bony surfaces, articular disc ,capsule ,ligaments to name a few - and unless u have seen these first hand you cannot identify as "Eyes only sees what the Mind knows-"

So for every Dr treating or managing TMD patient needs to understand what treatment would provide earliest and lasting relief and recommend treatments accordingly!!

*Can implants / Full mouth teeth cure Deafness*

My take -
A relatively lesser known ligament known as "Discomalleolar Ligament" which has high anatomical variation, attaches the articular disc of TMJ to the malleus bone of inner ear.

Now when patient has no teeth, the TMJ over roates in anterior region causing strech to the posterior disc region which in turn it may pull the above mentioned ligament and malleous bone causing deficiency in hearing!!

When after full mouth implants and proper vertical bite being restored the strech is reduced with proper discal position improving the hearing!!

Plausible?
Yes, but need to review whole case to verify!!

Now imagine a patient with improper bite, can the issue be vise versa - wrong bite affecting hearing??

Food for thought!!

PS : Above point os just a speculation of available information via a article in press, would really be exited to know the case first hand!!

*Importance of Proper Tongue Position*

A very very important point but probably neglected by Drs and patients, is proper tongue position!!

Normal tongue postion is behind upper front teeth, over palatal rugae with body of tongue resting on palate.

Many times during growing phase the tongue position remains unchanged as "infantile swallowing pattern" causing lot of issues with the jaw shape and size affect in the long term.
