Face represents the largest cortical expression on the brain covering nearly 35 to 40% - Homonculus.

Hence the face which is richly supplied by more than 6 cranial nerves, with bit of C1C2 division makes diagnosing the facial pain, its source and referral pattern difficult.

Sneek Peek into the TMJ Capsule!!

Very few Drs treating TMDs have actually visualised the joint intra capsularly -

Being fortunate to assist Dr Tofiq Bohra, who is a pioneer in Level 3 Arthroscopy in India, and being associated with him for about 100s of TMJ cases - which has lead me to realise that not all cases can be treated conservatively- And NO MRI can conclusively capture the damage done to the articular disc and retrodiscal tissues!!

Always while evaluating patients, we need have a mental picture of the joint, bony surfaces, articular disc ,capsule ,ligaments to name a few - and unless u have seen these first hand you cannot identify as "Eyes only sees what the Mind knows-"

So for every Dr treating or managing TMD patient needs to understand what treatment would provide earliest and lasting relief and recommend treatments accordingly!!

*Can implants / Full mouth teeth cure Deafness*

My take -
A relatively lesser known ligament known as "Discomalleolar Ligament" which has high anatomical variation, attaches the articular disc of TMJ to the malleus bone of inner ear.

Now when patient has no teeth, the TMJ over roates in anterior region causing strech to the posterior disc region which in turn it may pull the above mentioned ligament and malleous bone causing deficiency in hearing!!

When after full mouth implants and proper vertical bite being restored the strech is reduced with proper discal position improving the hearing!!

Plausible?
Yes, but need to review whole case to verify!!

Now imagine a patient with improper bite, can the issue be vise versa - wrong bite affecting hearing??

Food for thought!!

PS : Above point os just a speculation of available information via a article in press, would really be exited to know the case first hand!!

*Importance of Proper Tongue Position*

A very very important point but probably neglected by Drs and patients, is proper tongue position!!

Normal tongue postion is behind upper front teeth, over palatal rugae with body of tongue resting on palate.

Many times during growing phase the tongue position remains unchanged as "infantile swallowing pattern" causing lot of issues with the jaw shape and size affect in the long term.

Case of Trauma -
9 Yo/M reported with football hit on lips .
Parents visited within the half hour of trauma -

11 21 severly luxated - mobile vertixally in socket.
12 22 grade 2 mobile with mild lacerations on lips.

Immediate semi rigid fixation done with 016 SS round wire.

3 weeks follow up showed no mobility and complete socket healing.

What remians now to check for teeth vitality !!

*Temporomandibular Joint Disorder Management*

I often get many queries regarding 2 most difficult topics regarding TMDs-
First is how to arrive to a proper Diagnosis &
Second is what treatment options do we have for a particular diagnosis.

Unfortunately when managing TMDs we dont have many options available with us.
I have tried my best to summarize each modality in short in table below -

Rheumatoid Arthritis Vs Osteoarthritis

Amongst many joints that are affected by above degenerative joint diseases (DJD) TMJ, is no exception.

More than we thought and anticipated recent consenses shows that TMJ is more commonly affected by above 2 diseases.

Unfortunately identifying and diagnosing these disorders are quite difficult, before they show frank irreversible changes in the joints.

CBCT and Blood investigations along with case history forms the foundation to diagnose, the earlier the better prognosis of preventing the progression of DJD.
